Caught in, under or between a moving and a stationary object

The employee fractured the right thumb when it got pinched between two cables while using the crane and wrecking ball to scale down the west car dump pocket. The cable got hung up on a protruding stud and the employee attempted to move cable. The employee was wearing leather gloves at the time.

Employee was inspecting a lube pump and removed the cover. After the inspection the employee proceeded to replace the cover when it slipped out of EE's hands. While attempting to catch the cover the employee's right middle finger went under the seesaw mechanism causing a fracture to the tip of the right middle finger.

Struck by falling object

The employee was unblocking the 11 launder and lifted a grate. After pulling the second grate, the first cover landed on the employee's right hand causing a laceration.

While rotating a haul truck final drive during re-assembly per service manual instructions, two employees were using long pry bars to rotate hub assembly. One of the employee's bar slipped out of its position on the hub causing the employee to fall backwards to the ground, rolling EE's right ankle causing a fracture to the employee's right tibia and fibula.

A hose released pressure causing an abrasion to the left cheek. Employee said they were loosening and wiggling the hose and valve connection when it made a pop and they felt the oil hit to their cheek. Spray of oil happened and only lasted a split second.

Over-exertion in pulling or pushing objects

While pulling a 20' long 2""x2""x1/4"" stick of angle iron off a rack, the employee felt pain in their left elbow. The employee worked the rest of their shift, but then went to a local clinic on their own accord the following day for an evaluation without informing the company. This evaluation resulted in lost workdays.

While an employee was working to unload a rod car, the employee had a mis-step while transferring from one side of the car to the other and fell against the rods. The employee felt discomfort in their left side (rib area) and in their left shoulder. The employee later sought medical attention and was put on light restrictions.

Worker was assisting plant with pallet change out. Their hands were on the operating handle of the lifting device and the hoist was not centered over the load resulting in a swing and the lifting device came off the pallet. Adjacent pallets were in the work area and with the swing the worker's hand was pinched between one and the lifting device. One finger needed stitches

While an employee was working on a haul truck, dirt and debris fell from an above platform on the truck, some of which, landed in the employee's left eye. The employee sought medical attention. The eye was numbed, dirt or debris was extracted from the eye, and prescription eye drops were administered.

While lowering a removable mantle hand railing down on a mantle cart, the employee's left hand was in the line of fire, injuring two fingers. One finger suffered a laceration requiring sutures and another finger suffered a hairline fracture requiring a brace.

The employee scraped their left shin and twisted their right ankle. The employee was taken to a medical facility for evaluation and was diagnosed with mild sprain of ankle.

Employee states that while changing idlers on the 5/10 shuttle in the filter building, EE pinched their left pinky finger between the idler rack and the support beam.

Employee states was trying to line up a bar for the rail change. A co-worker was swinging a sledgehammer and missed the bar, hitting this employee in the nose. This was a tight area working next to grapple head, which was providing down pressure on rail.

Contractors were cutting holes in a section of chute in order to rig and remove it from the Pellet Plant Screen House. While cutting workers discovered a fire had started in the chute. Contractors notified their supervisor who then called 911. Fire department responded and extinguished the fire.

Employee slipped on a patch of ice while approaching a piece of equipment to talk with the operator. Employee landed on elbow which jarred the shoulder. Employee felt soreness in shoulder. Through an MRI a torn rotator cuff was diagnosed. Surgery to repair the shoulder took place on January 17th.

Employee states that they were attempting to travel down the ramp between 61/161 conveyor shuttles when employee slipped on pellets and fell backwards. As they tried to catch themselves with left hand, they fractured a wrist.

Standing on a 2' pedestal was going to grease a fitting on the motor, went to turn body and foot got stuck on grating and went over backwards landing on cement. Broke left hand.

Employee noticed a gradual onset of shoulder pain that they relates to demolition of overhead electrical conduit using a Sawzall. Ladders were utilized instead of aerial lifts due to tight spaces. First medical evaluation 5/9/19. Surgical repair was done 8/1/19. First time loss was 8/1/19.

Employee was setting a steel column and pulling up on a sleever bar when suddenly EE had pain in lower back. The employee did not report the incident till 1/14/19.

Employee states that while using a pry bar to lift idlers from the conveyor belt brackets, EE's finger was pinched between the bar and the conveyor frame. The initial force caused a fracture to the tip of EE's finger.

Employee was cutting steel bands from rod bundles when one of the bands sprung loose and contacted EE's lower leg. The result was a puncture wound that required sutures.

Employee entered elevator and pressed the 1st floor button. Elevator traveled downward and stopped. Control panel in elevator lost power and could not be moved up or down and the doors would not open. Employee had a power plant radio and called for help. Elevator repair company was called and replaced a failed mechanical relay. Once repaired, employee was able to exit elevator

Employee was using a impact tool to remove nuts and bolts from a loader bucket cutting edge when one of the bolt heads spun in their hand causing a laceration.

Employee states was pushing rags into a small opening to stop material from spilling from a rock box when felt a ""pop"" in finger. When employee removed glove saw that the tip of finger was bent funny and would straighten out.

Employee was using a crescent wrench to loosen a nut when the wrench slipped off the nut causing ee to strike finger against a hand rail support.

Employee was demoing existing duct work. EE torch cut a piece of 3/8"" steel plate that weighed approximately 30 pounds. The piece slipped out of left hand and struck steel toe on right foot. Right pinky toe was the only part of the foot not protected by steel toe where the piece fell. The piece of plate fractured the right pinky toe.

Contractor employees performing repairs at yard & docks were riding in the yard & dock elevator when it stopped. Elevator repair personnel from Thyssen & Krupp were called. The contractors were removed from the elevator, no one was injured.

Employee was injured when a sump that was being lowered into the basement with an overhead crane came into contact with a severely corroded pipe and broke the hanger causing the vertical pipe to fall and come into contact with the employee's ankle, fracturing it. The bolts on the hanger were completely rotten.

Employee was working to unplug a tails pump when the plugged material let loose and hot material sprayed EE's arm resulting in a 2nd degree burn.

Employee was aligning a 24"" carbon steel flange on a pipe. Flange slipped off the pipe and struck right heel of employee resulting in a fracture to his right heel.

Employee was riding in the Fine Crusher elevator when it stopped. Elevator repair personnel were on site and were unable to get the elevator to move. The employee was removed from the elevator car after a piece of tin was removed from the elevator shaft. The employee was uninjured.

Inattentive Driving: EE was a passenger in company vehicle driving on plant roadway. Driver reported sun glare caused him to veer and miss a turn in roadway. Continued down 5' steep grade into drainage area. The abrupt stop caused a fractured fibula bone to driver and passenger re-injured a previous non-work related injury to his wrist. Very little damage to company vehicle.

EE closing bin supply electric door. Wrapped the strap around hand and put weight into closing it. Was expecting door to close harder than it did since, door closed rapidly momentum pulled hand between closing doors causing hand to be lacerated by door latch upon doors slamming on hand. Laceration required 18 stitches, and there was no other damage to his hand still off work.

Employee did not report eye irritation until the start of his shift the next morning 09/06/13. Employee was taken to Eye Dr. where foreign body (dirt) was flushed out and given an antibiotic prescription as a precaution for his eye.

On Thursday March 15, 2012 at 2:45 pm, the employee was prying a head pulley over with a pipe. The work was being done on 163 Head Pulley. The pipe slipped and the employee slipped backwards. He fell on his back and his head.

Employee was removing a guard on some equipment with a 15 inch crescent wrench. The wrench slipped off of the nut and his right middle and ring fingers were pinched.

Employee was loosening nuts on the trunion of a ball mill. He was using a 16 lb. sledgehammer and 2.5 in. slug wrench. The hardware was very tight and he had hit one of the nuts several times with the sledgehammer. The last time he hit that nut, it loosened and he lost his footing. His body flew into the mill and he felt shoulder pain. That evening the pain worsened.

The electrical power source for the Yards & Docks elevator failed while an employee was riding in the elevator car. He was stuck in the elevator while the electricians worked to restore power. There were no injuries involved.

Emp sustained a laceration to the upper right forearm while performing overhead grinding with a 9"" angle grinder. Emp's arms were tired so he changed positions (had left hand on switch and right hand supporting head of grinder). The grinder kicked causing employee to lose control of grinder. Grinder fell and wheel contacted emp's right upper forearm causing laceration.

Employee was using a power hacksaw to cut a piece of steel pipe. As he reached to remove a hand broom from the cutting table, the saw blade contacted the broom handle and pulled his hand into the pipe. The result was a laceration to his finger that required sutures.

At approximately 2:00 PM on February 27th, a fellow employee was walking on the north side of 63/163 conveyor, when he slipped on the ice and hurt his back.

Employee was positioning the liner into place, and went to remove the liner clip, when a bolt that was holding the liner in place suddenly adjusted allowing the liner to fall slightly pinching finger.

Injured employee was struck on left arm by falling tool. Miner working above was using 2"" open end wrench, 24"" long, to tighten flange nuts on steam line. Lost grip on tool and it dropped 50' to floor below striking contractor employee sweeping floor. Resulted in 2 stitch cut to left upper arm.

Employee was inside rod mill when a liner fell stirking another liner. A small chip came off liner and struck him in the left calf. The metal chip went through the calf. He was treated at the scene and then taken to the clinic. He was checked out by the dr and returned to work.

Two employee were visually inspecting the fly ash system for possible material blockage. One of the ee lost his focus of the hazard that exited and reached thru an open valve to clean out 60 me accumulated flynsh. He received a fracture to the right thumb when the air operated value closed unexpected.

EE WAS REMOVING IDLER WHICH WAS STUCK. HE WAS HOLDING HIS HAND UNDER THE RETURN IDLER TO BALANCE IT WHEN IT FELL OFF FROM THE BRACKET BECAUSE OF THE MOTION FROM THE OTHER END. HIS LEFT MIDDLE FINGER WAS MASHED BETWEEN THE IDLER AND FRAME. LACERATION REQUIRED 3 STITCHES.

WHILE INSTALLING A NEW BEARING IN THE SCREEN HOUSE, EE BENT OVER TO PULL A SOCKET FREE FROM A BOLT. SOMETHING HIT A CAN OF ZEP SPRAY AND PUNCTURED THE CAN. SOME OF THE SPRAY HIT EE IN THE FACE AND EYES. HE WAS WEARING SAFETY GLASSES AT THE TIME.

While doing a liner job the person stepped on an air mover and fell backwards. In the process of falling the employee struck his hand on a sharp edge of the air mover.
