Individual was welding a Taper bushing that had broken out of a head pulley drum back into place. He was using a chain hoist to pull the shaft and the bushing back into place when the hoist slipped rotating the shaft that was attached to the drive pulley. Individual had his hand on the pulley and his thumb was pulled between the pulley and the drive belt.

Showing all 7Employee was helping tighten bolts on a cone crusher. He was using a 2 lb hammer and a wrench. While striking the wrench with the hammer, he missed the wrench and hit his 3rd finger on his left hand, breaking his finger.
Employee was using comealong on crusher to help free small debris when the cable broke and came back striking his left thumb and hand.
EMPLOYEE WAS USING A SLEDGE HAMMER AND STRAINED HIS LOWER BACK
EE WAS HELPING MECHANICS WORK ON JAW CRUSHER WHILE MACHINE WAS INOPERABLE. EE WAS USING A PRY BAR ON A LARGE BEARING WHILE THE MECHANICS WERE TRYING TO REMOVE THE SAME BEARING. THE BEARING CAP MOVED AND THE PRY BAR EE WAS USING SLIPPED, CUTTING HIS RIGHT HAND BETWEEN HIS THUMB AND FOREFINGER.
EE WAS TYING OFF A LADDER THAT HAD BEEN PLACED INSIDE A PUG MILL HOPPER. WHILE TYING OFF THE LADDER, EE PRESSED HIS RIGHT SIDE AGAINST THE HOPPER WALLS. WHEN HE DID THIS, HE FELT A "POPPING" IN HIS RIGHT SIDE.
EMPLOYEE WAS USING PRY BAR TO RE-POSITION A MASK PIN IN DRILL AND USING HIS OWN BODY WEIGHT TO PRY THIS PIN BACK INTO PLACE WHEN RIB FRACTURED.
