Showing all 5Employee was removing first slab from a block cut on belt using a crane and pry bar. The pry bar slipped causing employee to loose balance and fall against another block behind him. A slabs sharp edge came in contact with arm causing a laceration.
EE removing rusty bolts from piece of eq. by means of drilling. He used punch to indent bolt. He struck punch it fragmented and struck left earlobe. Small piece of metal removed from ear and rec. tetanus shot.
EMPLOYEE WAS ADJUSTING A RUBBER CUSHION ON A PUSHER BAR THAT ALIGNS TILE INTO THE BLADES FOR CUTTING. WHILE ADJUSTING THE RUBBER INTO THE SLOTTED BAR, THE RUBBER SLIPPED, THE FINGER GOT CAUGHT IN SLOT AND LACERATED RIGHT INDEX FINGER.
EE WAS WORKING ON AN AUTOMATED LOADING MACHINE THAT PICKS UP MARBLE SLABS BY SUCTIONED THE SLABS STUCK TOGETHER. EE TRIED PRYING THEM APART BY HAND CAUSING STRAIN.
