EMPLOYEE WAS (O5ERSEEING) REPAIRING A BENT SHAFT ON ROCK CRUSHER. THEY WERE ATTEMPTING TO STRAIGHTEN SHAFT WITH A PORTA POWER MACHINE. WHICH HE HAD BOLTED TO THE SHAFT. DURING THIS PROCESS THE BOLT BROKE AND 8TRUCK EMPLOYEE IN THE EYE. HE WAS STANDING APPX. 19' AWAY WHEN HE WAS INJURED.
