Showing all 4The employee was cutting a hole in the belt so we could put a rope through it to pull the belt together. He was holding the belt with his left hand and cutting it with a knife in his right hand when the belt slipped and the employee cut his left hand with the knife. The employee required five (5) stitches in his left hand.
Employee was performing normal duties. Brass coating on forks of forklift was damaged. He was repairing when incident occurred. Employee rotated the forks to the vertical position and was using a hammer to knock the brass coating back into place. The pin holding the top fork came out causing that fork to fall and catch employee's finger between the two forks.
