Miner dug out area between chute and belt. Miner looked into that area and a mass of material broke loose in chute and entrapped miner.

Showing all 26EE had symptoms of a cardiac arrest and was transported to the hospital. It was determined by a doctor that he did not have a heart attack and has Pleurisy, not determined to be work related. we reported this under the 15 minute rule.
EE was cutting a 24 inch I-Beam with a Torch. He backed up so another EE could knock it apart with a Cat 318 C LN Excavator with Shear attachment. When the other EE hit the beam it kicked up and fell on EE's leg.
He was cutting a plastic wire tie with a pocketknife when the knife slipped and stabbed him in the left forearm.
He was changing the grinding disc on a portable side grinder. He placed the tool between his legs without unplugging the power cord. His leg contacted the switch and the tool came on cutting his left fore finger between the first and second joint
EE was walking out the door on the kiln deck when dust blew into his right eye. EE did have safety glasses on. EE flushed the eye. Eye continued to bother EE until he went to doctor on 10/18/10. Diagnosed with corneal abrasion and conjunctivitis. EE put on light duty until 10/20/10. EE job is considered light duty so no changes in duty will take place.
He was stepping down from a catwalk to a conveyor belt when he hit his right knee on a pipe or beam. The doctor found a small fracture to his kneecap. He can work but no squatting , kneeling or crawling until released.
He was climbing down the ladder from the shovel. When stepping on to the ground he stepped on a rock and turned his ankle. He refused to go to the doctor and thought he was ok. On 6-11-2010 he said he was feeling pain and wanted to go to the doctor. The doctor diagnosed a class 2 sprain and ordered he wear an ankle brace for 6 weeks.
He climbed up on the front of a shovel track to get into the cab and hit his head on a mirror bracket. He did not use the step mounted on the machine and came up under the bracket from the wrong direction. The ER doctor gave him 5 sutures and said he suffered a concussion.He was seen again by the doctor on 3/9/2010 and cleared to return to work on the 10th.
Injured employee was cutting up a tree that fell down for firewood. The tree had poison ivy on it and the employee the next day started to show signs of poison ivy. Employee went to doctor on 10/10/08 and received a shot.
Injured employee was helping to fix a feed chute in kiln. Injured employee took off safety glasses and looked around chute. Another employee was using the torch to clean the area before cutting and blew dust towards the injured employee. Dust got in the injured employee's eyes.
He was helping 2 other miners move a piece of steel tubing onto a crusher cone liner, when it slipped and cut his left thumb, requiring 5 sutures. He was wearing gloves.
He felt hot, bed rested in scalehouse awhile, then went home. Later that night he went to the emergency room.
Employee was tightening screen bolts when she stepped through a hole in between the two screens in the catwalk. Employee's right leg went through an approximate 10 inch diameter hole up to her thigh. Employee recieved a puncture in the thigh that required 17 stitches. Employee is on restricted duty for 3 days.
Rock slid off highwall, rock fragment came through window of excavator and hit operator on the left shin. He did not need medical treatment at the time. On 1/29/07 he complained of swelling and went to dr. He is being treated for cellulitis.
Employee was hooking up a truck when the pressure in the line forced the cap off spraying product in his face.
Victim reported that smoke from diesel fuel filled his cab when a fuel leak occurred. He went to the emergency room on 7/29/06. From 7/30/06 until 8/4/06, he did not report for work. On 8/6/06 the events were reported to me. I investigated and reported to the insurance company. They notified me 9/21/06 that they were paying the claim.
The employee was cutting a hole in the belt so we could put a rope through it to pull the belt together. He was holding the belt with his left hand and cutting it with a knife in his right hand when the belt slipped and the employee cut his left hand with the knife. The employee required five (5) stitches in his left hand.
HE WAS GUIDING A PIECE OF METAL THAT WAS BEING COVERED INTO A SCREEN UNIT. HE DID NOT REMOVE HIS HAND IN TIME & CAUGHT HIS FINGER BETWEEN THE METAL & THE SIDE OF THE SCREEN. AVULSION TO HIS LEFT LITTLE FINGER, 11 STITCHES.
The kiln deck was being washed down and another employee was starting down the stairs, slipped and caught himself. When this happened the employee dislocated his shoulder.
Employee was performing normal duties. Brass coating on forks of forklift was damaged. He was repairing when incident occurred. Employee rotated the forks to the vertical position and was using a hammer to knock the brass coating back into place. The pin holding the top fork came out causing that fork to fall and catch employee's finger between the two forks.
A contractor was working on some demolition work at the mine site. He was standing on a catwalk with handrails cutting loose the bottom of the catwalk from a hopper. He cut both of the supports to the catwalk and fell approximately 23 feet. He showed signs of a leg injury and the hospital confirmed a broken femur and pelvis.
EE WAS CLIMBING STAIRS TO THE PRIMARY PLANT CONTROL TOWER, BOOT HEEL CAUGHT ON A STAIR TREAD, HE FELL AND CAUGHT HIMSELF WITH HIS LT. HAND.
WAS HELPING WELD AND INSTAL CHUTES AND GUARDS ON A CONVEYOR. REPORTED INJURY ON 4-1. DID NOT GO TO DR. REP[ORTED ON 5-8- THAT HIS BACK WAS HURT AGAIN. WENT TO DR. ON 5-8.
EE WAS HITTING A T BAR WRENCH WITH A SLEDGE HAMMER THE HAMMER HIT THE WRENCH A GLANCING BLOW & STRUCK THE EE IN THE RIGHT ANKLE. INJURY WAS AFRACTURE TO RIGHT ANKLE AT TIME OF ACCIDENT INJ WAS WORKING ON CONE CRUSHER
TRUCK DRIVER WAS HOLDING TAILGATE OPEN ON STOCKPILE TRUCK. HAND SLIPPED AND MASHED LEFT MIDDLE FINGER, FRACTURE TO FINGER.
