Showing all 11Employee was cutting belting with a utility knife. The blade became hung up on the belt clips causing the knife to skip and make contact with the employees L lower forearm causing a laceration requiring sutures.
EE WAS SETTING TIMING ON THE SCREEN DECK SHAKER WHEELS. EE PUSHED ON A WRENCH TO MOVE THE WHEEL INTO TIME AND EE'S SHOULDER STARTED HURTING VERY BADLY SO EE STOPPED PUSHING.

Employee was inserting metal pin into a wear part on a bucket for a hydraulic excavator. Employee struck thumb with hammer.
Working on a heavy hydraulic cylinder and when ee tried to rotate it the cylinder slipped towards ee legs so ee grabbed it and a jagged edge pinched ee fingers against the steel on the main equipment.
Employee was using a hammer to wedges in at a new tunnel being built. EE missed the wedge and struck left thumb, resulting in swelling and tenderness.
Sledgehammer came out of nearby employee's hands, struck an object, which changed its direction. It came around toward the other employee. The wood handle barely hit his head. He received three stitches from the closest clinic for a small cut.
Employee was removing filter canister when it broke loose and wrench turned and pinched finger between wrench and frame of machine.
He was trying to remove a clamp rod and the pipe wrench he was using broke. Wrench snapped back and broke his left index finger.
