Employee was cutting plywood with a 7-1/4" Circular Saw when their left hand was placed too close to the operating saw. The blade struck their left hand pinkie finger severing the tip of the finger just past the last knuckle closest to the finer nail.

EE WAS SETTING TIMING ON THE SCREEN DECK SHAKER WHEELS. EE PUSHED ON A WRENCH TO MOVE THE WHEEL INTO TIME AND EE'S SHOULDER STARTED HURTING VERY BADLY SO EE STOPPED PUSHING.

The fines box on the jaw crusher was plugged with rocks. EE was digging out the rocks and cleaning up the cat walk. EE stepped on a rock on the catwalk and rolled an ankle which broke their foot.

Working on a heavy hydraulic cylinder and when ee tried to rotate it the cylinder slipped towards ee legs so ee grabbed it and a jagged edge pinched ee fingers against the steel on the main equipment.

EE was operating the primary crusher and noticed a large steel wheel coming through the feeder. He stopped and locked out the crusher, put on a harness and climbed into the feeder. While removing the steel wheel, he felt what he described as an electrical shock from the lower back to the base of his neck. He restarted the crusher and informed his supervisor of the injury.

EE was working on a rock box on a conveyor and remained on the catwalk afterward to make sure everything was working properly. A rock bounced on the nearby handrail and hit him in the knee and thigh.

The employee was assisting with changing screens on a portable screen plant. Two employees were handling one screen panel weighing approximately 50 pounds when one employee felt pain in lower abdomen. Employee was sent to physician for a check and was diagnosed with an Inguinal Hernia. Consultation with a general surgeon was scheduled and he returned to work on restricted duty.

Employee was changing screens on a screen plant when he bumped his left leg on the corner of a screen panel. The employee continued working because he had no pain or injury from the bump. One week later, on 5/27/2014, the injury was reported to us after a weekend visit to the doctor for pain and swelling on the left leg where it was bumped.
