Showing all 5Employee was using a two pound hammer to hit a frozen/stuck pin on a skid loader so the attachment could be changed. When hitting the pin some debris entered employee's eye. Employee had to have debris removed from eye. Safety glasses were worn.
Employee struck thumb on right hand with hammer while removing a speed reducer on a lime belt.
Cone had bridged over, employee's were using pry bar to dislodge and clear out cone. Injured was using bar to pry rocks loose and clear a hole. The bar rebounded back and struck him in the palm of left hand.
Installing a new ware liner in shaker 3 collector discharge chute. Swinging 3lb hammer through opening in door to make sure ware plate was secure. Caught corner opening of door with hammer, causing it to ricochet over and hit right thumb. Thumb was broke - cracked by knuckle and chipped by joint.
