Accident type, without injuries

A 8,000 gallon diesel fuel tanker with an estimated 1,000 gallons or less in it when it caught on fire. A gasoline powered engine ran the fuel pump on the tanker. The fire dept. was called and the fire was extinguished in approximately 45-50 minutes. The investigation was unable to reveal the exact cause of the fire.

Struck by flying object

A 2 cm by 1 mm piece of metal struck EE in the inner thigh and traveled about 3 inches upward. The metal had to be surgically removed.

EE WAS CLIMBING UP INTO 228 CAT LOADER 950 AND HE SLIPPED AND FELL STRIKING HIS RIGHT HAND ON THE WAY DOWN AND BROKE HIS RIGHT HAND. HE HDA SURGERY AND PUT STEEL PLATE IN HAND AND BACK TO WORK NEXT DAY.

WHILE LIFTING A BARRELL OF OIL HE FELT A PULL IN HIS GROIN AREA BUT DID NOT THINK MUCH OF IT UNTIL LATER.

GETTING OUT A TRUCK, HE GOT PINKY FINGER CAUGHT ON THE DOOR AND CUT IT. HE REFUSED A TETNUS SHOT AND SUTURES BECAUSE HE WAS AFRAID OF NEEDLES.

WAS WORKING ON A MAGNETIC STARTER AND IT EXPLODED RESULTING IN A BURN TO THE RIGHT HAND.

EE WAS USING A SIX FOOT CHEATER BAR AND WRENCH TO CHANGE A TIRE, THEY WERE PULLING ON BAR AND WRENCH WHEN IT SLIPPED AND EE FELL BACK AND HIT THE TRUCK CUTTING HIS RIGHT EYE.
