EE was cutting a piece of conveyor belting with a utility knife when the knife slipped stabbing the EE in the right thigh just above the knee. The EE received 3 stitches making this accident reportable to MSHA.

EE was tightening the packing on a slurry pump. The wrench slipped causing EE's hand to hit the main frame of the pump. The top of the little finger on the right hand was cut, requiring 2 stitches.

EE was cutting a piece of thick skirt rubber with a box cutter. The box cutter slipped and cut thumb on ee left finger, requiring three stitches, thus making the accident reportable.

EE was hammering some rust off of some metal in a bin in which he was getting ready to repair. While hammering, some type of debris got in his eye (he was wearing safety glasses), and extremely irritated left eye. Went to doctor, but there was no debris in eye, just irritated. Doctor gave prescription ointment for eye, thereby making it reportable.

EE was removing bolt from Sheave while in a vice. The EE pulled on the bolt with a wrench, the sheave slipped in the vice and pinched the EE's left small finger between the sheave and the vice.
