I.E. was assisting in changing belt rollers in the slope. As I.E. released the come-a-long, left middle finger was caught in the chain. It cut the tip of the left middle finger off. I.E. went to Dr. & was treated. I.E. came back to work on next scheduled shift & has not missed any work due to this accident.
Face chain had broken & they were trying to put it back together. They had a com-a-long holding the tensioner motor down. The operator engaged the tensioner but put too much pressure on it & the com-a-long chain broke & hit the I.E. in the nose & right side of face. It fractured their nose & safety glasses pushed up & got stitches above the eye.
We have contracted GMS to help with set-up of our new longwall. One of their employees went to the return to use the bathroom & didn't have any toilet paper so they used a knife to cut a shirt to use instead. They cut the left thumb trying to cut the shirt to use for toilet paper & got 3 stitches. They returned to work their next shift & missed no time.
EE was assisting with realigning the bearings on the slope belt with a come-a-long, when EE felt a strain in EE's lower back and abdomen.
Employee was adjusting rub-rail on miner with rock bar to insert bolt in top. When employee lifted upon rail causing a pain in lower back.
Injured was operating scoop, assisting in putting cat tracks on the miner. A come-a-long was being used to pull the cat track. The come-a-long gave way and struck injured in the head requiring stitches.
Injured was assisting make repairs on CM 22 when a piece of small flake of chisel impaled in the injured's left arm.
Injured was prying on a cutter shaft when the rock bar being used slipped, striking injured in the face. Injured required 10 stitches just above lip.
Injured was shoveling out the inside of a rail slag car when the injured's left foreman struck against a metal burr on the car side causing a laceration requiring stitches.
Injured was removing a screw jack from the slope belt when the jack came off prematurely striking the injured's foot causing a fracture to the foot.
Employee was cutting bags open to fill a rock duster. His hand slipped and he cut his palm.
While attaching come-a-longs to a chain the hook slipped off hitting the employee on the head requiring stitches.
Employee was swinging a sledgehammer to drive in a final drive motor when the employee felt pain in the right arm. Injured was diagnosed with a elbow sprain.
Injured was splicing a feeder cable and was down to the support strips when the knife slipped striking the left middle finger resulting in 7 stitches.
Injured was jacking a rail mounted rock duster back onto the track when the duster slipped and caused injuries to the left hand.
Injured was jacking a wrecked manbus when the left middle finger was caught between the jack and the coupler of the manbus when the manbus shifted. The finger required surgery to repair the damage to the tip.
Employee was standing next to where his buddy was using a hammer to knock a link apart and a piece of the pin on the master link flew into his left lower jaw.
While raising door with rock bar employee felt a pop in lower back causing a strained back.
While installing grid coupling cover using a hammer the hammer glanced off striking the employees left middle finger.
Employee cut his left thumb while removing outer jacket of cable. He was wearing cut resistant gloves.
Employee was skinning cable back to splice leads together and cut his thumb requiring stitches.

A come-along was hooked to the cutter head to hold it in place. The come-along broke and the hook hit the employee on the right forearm.
injured was striking metal conveyor chain when a chip flew off striking injured in his left forearm. chip was imbedded in arm and removed by physician.
EE placed chain fall onto conduit above/behind him. Chain fall fell and struck EE in the back of head. EE reported the incident on 12/18/11 to myself, EE stated that he would wait until after the holiday to see how his neck felt before going to the doctor. EE returned to work 12/26/11 and was taken to seek medical treatment and was released back to work on light duty.
Employee hit his hand with a hammer while trying to straighten a pin on the booster drive.
Employee was cutting skirt rubber with a razor knife when the knife slipped striking the employee in the right hand requiring 2 stitches.
Injured was holding a wedge while another employee attempted to strike the wedge with a hammer, subsequently missing striking injured in the left middle finger causing a fracture.
Individual was rolling a track rail with a slate bar when the bar came loose striking employee on the right side of the face. The injury required 12 stitches.
Injured was needing to cut banding material off of a pallet and did not have band cutters. Injured then broke the band with channel locks when the band broke it cut the right hand requiring 3 stitches.
Injured was cutting the outer jacket from a cable. Injured was using a razor knife. The knife slipped striking injured on the tip of his left index finger resulting in a laceration requiring 5 stitches.
Injured was in process of installing a d-5 pump at the south dams. A screwdriver he was using spun around and caught his left hand resulting in a puncture wound. No stitches were required and injured was released for work. (This injury was not determined to be reportable until subsequent evaluations as non medical issues were discovered and are still pending.)
Injured was using a pry bar to remove a stuck bottom roller from the belt line. The bar slipped catching injured's thumb between the bar and roller frame, resulting in a fracture.
Injured was cutting a belt when the knife slipped cutting his left arm.
Injured was putting a wrecked mantrip back on the track. Injured was using a jack and bar to lift the mantrip. The jack slipped causing the jack bar to strike injured in the nose.
Injured was using a sledge hammer to drive a coupling back onto a motor shaft. As injured swung the hammer he felt pain in his lower back. (Note; company was not notified injured was off work due to this injury until 02/27/08)
Injured was removing a staple lock fitting staple. Injured was using a hammer and chisel to knock out the staple. A piece of metal from the hammer came off. The metal piece went under injured's safety glasses and struck him on his right cheekbone and right eye.
injured was using a pair of come-a-longs to pull open the northgate of the sump hopper. Injured felt a pain in his right shoulder.
INJURED WAS USING A UTILITY KNIFE TO CUT OFF A TORN PIECE OF CONVEYOR BELT FROM THE SECTION BELT. THE KNIFE SLIPPED AND CUT HIS RIGHT LEG REQUIRING 4 STITCHES.
injured was using a ratchet, socket and bolt tap to rethread bolt hole. Injured removed gloves to start tap in the hole. His left hand was on the ratchet head holding it on the tap and right hand pushing down on ratchet handle. The tap shank broke causing injured's left hand to move suddenly and strike his left forefnger on the broken tap resulting in a cut requiring stitches.
injured and co-worker were using a pipe wrench to free a pump sheave wheel. the sheave wheel suddenly free up causing injured to pinch his right ring finger between the sheaved wheel and the pipe wrench.
The injured and co-workers were in the processing of removing conveyor belt from the belt takup. The injured was using a knife to cut the excess belt. The knife sliped sutting his left small finger. The cut required 4 stitches. The injured person was wearing gloves at the time of the accident.
Injured was using a 16# sledge hammer to break rock at a rock fall. injured was swinging sledge hammer and felt pain in his arm. (Injured did not request medical treatment until 04/28/06)
INJURED AND CO-WORKER WERE TAKING A CANOPY OFF A 630 SCOOP. THE INJURED EMPLOYEE WAS UNDER THE SUSPENDED CANOPY. WHEN THE SCOOP WAS MOVED, THE CANOPY BECAME CAUGHT ON THE SCOOP TIRE FENDER. CO-WORKER USED A BAR TO LOOSEN THE CANOPY CAUSING IT TO SHIFT AND STRIKE INJURED IN THE HEAD.
INJURED WAS RETRIEVING A SHOVEL FROM UNDER A PILE OF ROCK. THE SHOVEL HANDLE BROKE AND STRUCK INJURED ON HIS LEFT FOREARM RESULTING IN LACERATION.
INJURED WAS USING A PRY BAR TO SUPPORT A SPROCKET. THE PRY BAR SLIPPED CAUSING INJURED TO CATCH HIS RIGHT THUMB BETWEEN A STAGELOADER CHAIN LINK AND THE PRY BAR. THIS RESULTED IN A LACERATION REQUIREING 5 STITCHES. INJURED WAS WEARING GLOVES AT THE TIME OF THE ACCIDENT.
Employee claimed back pain next day after using chain falls on 10-28-04.
Injured was using a 3/4 ton come-a-long to couple carrier to a locomotive. The come-a-long spun around catching his finger between the handle and the chain.
injured was using a come-a-long to remove the door from the longwall head drive. the door fell on injured leg.
The injured was using a knife to cut a hole in a piece of belt. The knife cut through the belt and into injured left wrist. This resulted in a cut requiring approximately 16 stiches.
