I.E. was assisting in changing belt rollers in the slope. As I.E. released the come-a-long, left middle finger was caught in the chain. It cut the tip of the left middle finger off. I.E. went to Dr. & was treated. I.E. came back to work on next scheduled shift & has not missed any work due to this accident.

Face chain had broken & they were trying to put it back together. They had a com-a-long holding the tensioner motor down. The operator engaged the tensioner but put too much pressure on it & the com-a-long chain broke & hit the I.E. in the nose & right side of face. It fractured their nose & safety glasses pushed up & got stitches above the eye.

We have contracted GMS to help with set-up of our new longwall. One of their employees went to the return to use the bathroom & didn't have any toilet paper so they used a knife to cut a shirt to use instead. They cut the left thumb trying to cut the shirt to use for toilet paper & got 3 stitches. They returned to work their next shift & missed no time.

EE placed chain fall onto conduit above/behind him. Chain fall fell and struck EE in the back of head. EE reported the incident on 12/18/11 to myself, EE stated that he would wait until after the holiday to see how his neck felt before going to the doctor. EE returned to work 12/26/11 and was taken to seek medical treatment and was released back to work on light duty.

Injured was in process of installing a d-5 pump at the south dams. A screwdriver he was using spun around and caught his left hand resulting in a puncture wound. No stitches were required and injured was released for work. (This injury was not determined to be reportable until subsequent evaluations as non medical issues were discovered and are still pending.)

injured was using a ratchet, socket and bolt tap to rethread bolt hole. Injured removed gloves to start tap in the hole. His left hand was on the ratchet head holding it on the tap and right hand pushing down on ratchet handle. The tap shank broke causing injured's left hand to move suddenly and strike his left forefnger on the broken tap resulting in a cut requiring stitches.

The injured and co-workers were in the processing of removing conveyor belt from the belt takup. The injured was using a knife to cut the excess belt. The knife sliped sutting his left small finger. The cut required 4 stitches. The injured person was wearing gloves at the time of the accident.

INJURED AND CO-WORKER WERE TAKING A CANOPY OFF A 630 SCOOP. THE INJURED EMPLOYEE WAS UNDER THE SUSPENDED CANOPY. WHEN THE SCOOP WAS MOVED, THE CANOPY BECAME CAUGHT ON THE SCOOP TIRE FENDER. CO-WORKER USED A BAR TO LOOSEN THE CANOPY CAUSING IT TO SHIFT AND STRIKE INJURED IN THE HEAD.

The injured was using a knife to cut a hole in a piece of belt. The knife cut through the belt and into injured left wrist. This resulted in a cut requiring approximately 16 stiches.
