Employee struck a piece of metal with a hammer causing a quarter inch shard of metal to chip off and embed itself in the top on EE's right hand. Employee was wearing PPE to include work gloves but the piece of metal pierced through the top of the glove and into the back of EE's hand. Stitches were required after removal of metal shard.

Employee was observing a coworker remove a stuck tooth from dragline bucket with a hammer, when a piece of metal from the tooth dislodged and embedded in injured's left wrist while walking away from jobsite causing a puncture wound.
