Employee was replacing head pulley bearing when employee's finger was pinched against the conveyor frame

EE was operating loader when EE drove over uneven ground causing the loader to move side to side aggravating a pre-existing lower back condition from 6 years prior.
As employee was stepping off the back of mechanics truck, they rolled their ankle/leg resulting in a closed fracture to the fibula just above their right ankle.

Employee struck a piece of metal with a hammer causing a quarter inch shard of metal to chip off and embed itself in the top on EE's right hand. Employee was wearing PPE to include work gloves but the piece of metal pierced through the top of the glove and into the back of EE's hand. Stitches were required after removal of metal shard.

Employee was attempting to step up onto a seven inch high steel grated walkway and caught the toe of boot causing her to fall forward and dislocate her left fifth (pinky) finger.

EE suffered a heart attack while at the aggregate plant. EE was taken by ambulance to the hospital. He was pronounced dead at the hospital.Ee had been treated recently by his personal physician for heart-related health issues.EE was using an inhaler medication to help him breath.
