MMG's were removing the 5B rougher drum in the east side of the Concentrator to repair the tub. They used OH crane to remove drum to set the magnetic rougher drum on ball mill floor cover. The cover became stuck to bottom of drum. MMG jumped on 5ft pry bar placed between cover & drum (@ angle ~4-5" from ground), when cover released, EE injured right foot.

Employee was using brass punch to remove rod screen arm bearing. During this activity a piece of the punch broke off hitting employee's tooth. The employee did not visit a dental office until 9/10/25, and at which time, the tooth was repaired. The office visit summary wasn't received until 9/16/2025

Employee was using hammer and chisel to remove a carbide insert when a piece of metal broke from the chisel and punctured the inside of the left wrist and becoming embedded beneath the skin.

EE was using a pocketknife to cut a zip tie holding a pair of cut resistant gloves together. The pocketknife cut through the zip tie and contacted the EE's left middle finger resulting in a minor laceration. Two sutures were required.

EE was removing a bearing race off a shaft. The race was first cut with a grinder and then the EE attempted to split the race with a chisel and hammer. While removing the race, a small piece of shrapnel struck the employees left palm. Medical treatment was required to remove the foreign object on 5/14/2018.

Employee was using a 2' sleever bar to align bolt holes. Sleever bar slipped out of bolt hole causing employee to fall to right. Employee struck guarding latch below the employee with left arm pit at the same time employee's fall protection caught. Employee sustained a cut to left arm pit needing stitches.

A non-employee trespasser was found dead hanging in a tree by a city of Eveleth employee. Death was ruled a suicide by ligature hanging. This is an inactive area of the mine adjacent to the City of Eveleth. Eveleth Police Dept. ICR# 17086466.

The EE was installing a wing shroud on a dozer. While attempting the pound the keeper pin in place, the ee hit the pin and glanced off hitting left index finger with the hammer. The ee was sent into the clinic and received two sutures. Risk 1. Low

A piece of channel iron got caught on expanded metal. The worker used a Johnson bar to pry the channel free. The channel sprung up and struck arm causing a laceration.

EE was installing a 3/8" tubing adaptor with a 3/4" combo wrench in a tight awkward position. As EE was torquing the adaptor into place the wrench slipped & EE's hand hit the orifice plate handle on the orifice plate on the primary air.

Injured EE was tying a piece of rebar back into place after moving it to clear a concrete wall form. As EE was pulling the tie wire tight, the pliers slipped free, striking EE in the upper right side of mouth/face. This resulted in the upper, second tooth to the right from center, tooth being chipped and loosened.

The EE was removing a pin from a bore on a haul truck. When the pin cleared the bore it shifted downward pinching the mechanics thumb between the tool and the pin resulting in sutures.

Injured employee was working with partner installing Classifier flights. She was using a 3/8" allen wrench to hold bolt head while partner used a 3/4" impact gun to tighten nut. The nut seized causing the allen wrench to rotate resulting in the employees index finger being pinched between the wrench and the flight shoe. The distal 3/4" of the right index finger was crushed.

Injured was removing section of cast liner in side rod mill. As he was striking the bottom of the casting to break it loose an approx 12"x18"x5" piece of the top of the liner broke off and fell approx 4 feet striking the employee on the top of his left foot. The piece weighed an estimated 75 lbs. Employee was wearing work boots with metatarsal protection.

Employee was loosening case mount bolts from the frame to the case on 3C tails pump working in a bent over position he felt a strain in lower back. He was using a 3/4 inch ratchet with a 2 1/4 inch socket to loosen the bolts. Did not seek medical treatment (physicial therapy prescribed) until 11/16/09. Did not notify United Taconite of medical treatment until 11/17/09.

Unit 26 was replacing cracked turtle(cover plate) on 18 shovel, the shovel operator was assisting unit 26. Unit 26 noticed the operator's face /cheek had some blood on it. The EE was transported to the foreman's office and the 1st responder transported EE to E.R.

Employee repairing ball charging hose on 2A ball mill grabbed a 12 lb hammer to knock wedges out while doing so felt a pop in low right back. On 4/10/08 received Rcvid physician report of workability stating restrictions from 4/2/08 through 4/9/08. Restrictions ended as of 4/9/08. Employee did not work on 4/8 or 4/9 as these were normal days off work.

Employee felt muscle tightness torquing head bolts on rear engine of 9017 shovel. Felt tightness and soreness in arm next morning. He received diagnosis on 7/20 tendinitis and will start physical therapy next week for several weeks. He is also missing time as of 7/24/08.

EE WAS WORKING ON 9483 WITH COWORKERS, ATTEMPTING TO REMOVE PIN FROM SURE-STRIKE ATTACHMENT WHICH WAS OFF THE UNIT. WHILE EE WAS WALKING AWAY TO GET A LARGER HAMMER, A COWORKER CONTINUED TO HIT THE PIN WITH A HAMMER. A SMALL STONE CAME OFF THE PIN AND STRUCK EE IN THE KNEE. REQUIRED STITCHES.
