Driving a shear pin in conveyor chain on feeder with hammer, the hammer slipped and finger caught the frame of the feeder. Was taken for treatment received stitches in his index finger.

He was checking for what side of the beltline was the thick side. The knife he was using slid off the beltline as he was cutting it. The knife came in contact with the palm area next to his thumb on his left hand resulting in a laceration.

EE was using a slate bar to free a canopy out of the mud, when he felt a burning in his lower back, at the time it happened EE indicated that he felt fine. He started missing work from the incident on 09/07/2010.

EE was loading conveyor belt on a belt car and was using clamp (rope dog) to pull on the belt. The clamp slipped off the belt, causing an injury to his left shoulder. Possible rotary cup damage to shoulder. He started missing work from this incident 09/07/2010.

Doing repair work on electric roll up door at mine portal when the wrench slipped off a bolt hitting his left hand on the edge of a metal enclosure causing a cut on the back side of left hand. This required 4 stitches to the left hand.
