Employee was stripping ground cable with a pocketknife. Employee was not wearing gloves at time of the incident.

Employee was trying to free a stuck rigging hook on shovel dipper door. Employee used a sledge to try and free the hook. Using both hands to swing the sledge, employee freed the hook which in turn swung back and struck the employee's right thumb.

WHILE STRIPPING 4/0 INSULATION THE KNIFE SLIPPED SIDEWAYS OFF THE END OF 4/0 ELECTRIC CABLE (WHERE THE LUG GOES) AND CUT THE LEFT PALM BELOW THE THUMB. EMPLOYEE WAS WEARING CUT GLOVES. EMPLOYEE REPORTED TO AMES SAFETY AND WAS TAKEN TO FMI RESCUE FOR EVALUATION. EMPLOYEE'S SUPERVISOR TOOK EMPLOYEE TO A CLINIC FOR FOLLOW UP CARE.

TECH WAS REMOVING BUCKET TEETH AND TOOTH WAS STUCK. TECH USED HAMMER AND HIT THE TOOTH. A PIECE OF FLY METAL BROKE LOOSE AND HIT TECH IN THE LEFT KNEE. TECH FINISHED WORKING THE SHIFT.

While employee was performing their daily task installing a bracket on a repeater pole, they struck their left hand with a 4-pound sledgehammer, causing swelling.

The driller was using a hex tool wrench to remove a 2" pipe fitting from the side inlet body. While pushing hard to break the fitting, the wrench slipped and EE's hand struck the drill mast. Wrong tool for the job was used. Driller had used a Hex tool wrench in place of a Pipe Wrench. Pipe wrenches ranging in size from 18 to 36" were already on site. Incorrect Safety Glove worn.

Employee cut their middle finger on a sheet of copper while attempting to remove an imbedded pre-strip clip from a bundle with a crowbar. The laceration required two stitches.

While working on Drill 14, the employee was working out of a man basket attempting to remove a pin from a cylinder. The hammer got caught on the safety cable for the JLG controls causing the magnetic base to become loose and strike the employee in the left eye. This caused a small laceration.

Employee was removing weeds when EE felt pain in EE's left shoulder after putting pressure to up root a bush with the shovel.

Employee was removing material from behind the kingpin on the horizontal portable when material sluffed down hitting employee's legs, knocking employee down. Employee hit right lower back on the structure behind employee.

Employee was dismantling an old screen using a pry bar. While attempting to loosen a lower piece of angle iron, another piece of angle iron fell from higher section of the screen, striking employee on left wrist.

Operator was unplugging a feeder using mental bar when the bar slipped and pinched EE's right pinky finger causing a laceration and fracture.

A mechanic was cutting a zip tie off a wire harness with a razor. When EE broke the zip tie the blade slipped, striking EE's hand. This resulted in a laceration between the left thumb and pointer finger. The employee received 8 stitches and was restricted from using EE's left hand.

Employee was removing pre-strips from stainless steel blanks with 3-foot pry bar utilizing a bat swinging motion when right pinky finger came in contact with a pre-stripped piece of copper.

Mechanics were using a steel hammer to remove old teeth from a 994 loader bucket. A piece of fly metal came off the hammer and entered the employee's right forearm.

Employee was using a squeegee to pull mud and electrolyte solution into a vac truck pipe and some solution splashed them in the right eye.

Employee was stopping a liner bolt from spinning with a pry bar when it slipped causing ee to cut finger on the spinning bolt.

An employee lost control of a 4lb hammer. Striking the injured individual in the left knee. The individual experienced tenderness in the left knee resulting in restrictions to work duties.

Employee was using a hammer and drift pin to drive inserts out of a crusher head nut. Employee missed the pin and struck the back of EE's right hand. EE required five sutures to close laceration.

Field Helper was getting EE's equipment ready for the shift. EE was attempting to cut a pulling strap with EE's knife. EE's knife went through the strap too quickly and made contact with EE's hand. The injury required medical attention and stitches to close the wound. EE was cleared to return to work with no restrictions.

Employee was using pry bar to dislodge decking plate. Decking plate broke free and ended up pinning the pry bar, which resulted in pinning employee's hand between pry bar and side of chute.

Employee was using a j-hook tool to push a blank back and felt pain in right hand and wrist.

Employee was working on a 793B-3516 engine, removing coolant tee on engine. Loosening mounting nut with wrench, when the nut broke loose wrench slipped & left ring finger with wrench in hand struck open coolant port, breaking tip of finger

EE was hammering on the bottom of a rail car and reported had something hit eyebrow then went into eye. EE was taken to the clinic.

The mechanic was using a pocket knife to cut rubber skirting. The knife slipped and cut ee left forearm which required five stitches.

Employee was cutting twine off two inch corrugated airlines and cut lower leg in process.

Employee was cutting nylon rope with a utility knife. The knife slipped causing a laceration to the parties left middle finger.

Employee attempted to dislodge a pry bar from a plugged chute with his right foot. The pry bar gave way resulting in an ankle fracture.

Employee was removing ground cable at the E3 mixer, while he was cutting he tie straps off with a pocket knife he cut his left middle finger.

While employee was removing zip-tie with pocket knife tool, the blade slipped off and cut his left hand.

Employee was changing the cutting edges on Motor grader #4, When one of the bolts wouldn't come out, the employee used a punch and hammer to remove the bolt. The employee struck his right index finger with the hammer resulting in a laceration that required two stitches.

Employee was installing a 4" clamp onto pipe. He then hit the wedge on the clamp with a 4lbs. hammer causing the wedge to break and strike him in the mouth.

Employee was pulling screen panel with a 1 Ton come along chain hoist. As the screen panel pulled free from its bindings, the employee continued to crank the handle of the hoist. When the screen panel finally pulled free, it did so suddenly, causing the employee to over pull the handle and his elbow made contact with the screen.

Fracture to right pinky knuckle.

Employee was adjusting liners inside feeder chute. While using a 3 lb hammer to raise the liner by hitting the picking eye, the picking eye broke loose and struck the employee in the mouth.

While performing normal job duties, EE experienced normal soreness in his left shoulder that did not go away for a couple of days. Two weeks later while cutting ACSR wire, EE experienced a burning sensation in left shoulder.

While a mechanic was hammering a track pin on Shovel 3, a fragment of metal dislodged from the pin and embedded into the upper leg of an employee who was standing approximately 3-4' away.

Two maintenance employees were raising liners on a discharge chute which was on ground level. One employee was positioned inside with a four pound hammer while the other employee was outside with the impact drill. The employee inside began to hammer on the liner, the employee outside noticed that it was stuck and positioned his hands to push when his left index finger was struck.

Employee was driving 12" rebar stake to support electrical conduit for concrete pour, when 10 lb.sledge hammer slipped off rebar coming in contact with left thumb, causing a laceration and a tuft fracture.

Employee was cutting a piece of rubber belting for the tail chute of the conveyor. The belting was placed on the ground on top of some 2 x 4's. The employee was on his knees cutting towards him. The knife blade cut through the belting and punctured the employee's leg.

Employee was assisting with cutting strips of conveyor belt. He knelt down and positioned himself such that the utility knife would be cutting towards him. The utility knife hung up on the belting as he was cutting and so he put more force to dislodge the knife and as he did it broke free and the employee stabbed himself in the leg.

The employee was replacing liners in the bin. The liner was hot tacked in place when he struck it with a 4-pound hammer to further position it. When the 4-pound hammer contacted the liner at a glancing angle instead of straight on, the hot tack broke and was projected behind the safety glassess into the left eye.

Employee was repairing a 3/4" plug on a 4" HDPE pipe while on Cell 12 of the Leachpad. Employee was trying to tighten the 3/4" plug with a pair of channel locks when it sprayed raffinate towards his face.

Employee was changing picks on grader. When the employee swung his hammer it glanced off the pick. The employee's thumb contacted a burr on the pick lacerating his hand.

Trying to extract a bolt from hoist drum with the hammer and bolt, when the hammer slipped off the head of the bolt and hit his little finger.

Two employees working with end wrenches. One employee's finger was pinched between the wrenches.

The cathode lifting tool that the employee was using to realign the conveyor chain slipped off the chain and struck the employee on the chin.

Employee was loosening a bolt on the 2280 conveyor when his wrench slipped resulting in a finger laceration.

Employee was assisting a crew installing a bushing on a 2800 shovel. The employee used a hammer to strike the bushing to help drive it in place. When he struck the bearing (two hardened surfaces) a small chip of steel broke off and embedded in his knee.

Employee was cutting an electrical cable tray and the Metabo kicked back and hit his hand.
