While backing up haul truck to shovel, employee had neck pain when the truck struck the shovel dipper which caused the contents to drop in truck bed.

While attempting to make a left turn in the company LV335 flatbed truck, it was hit by a personal vehicle on the driver side door. The impact caused the employee to hit their head on the driver side window.
Employee struck driver's side door of company vehicle with personal vehicle. Front air bag deployed. Employee has reported neck pain on 2/24/26. Examined at Health clinic on 2/25/26.
while attempting to make a U-turn on the UTV, the UTV turned onto its side. This resulted in injury to lower back, right flank and right hip.

The employee was leaving EE's Shovel 6 assignment when EE made contact with a pothole in the haul road. The Employee states EE has strained EE's lower back pain.
Crane Operator was preparing the rig to be strapped to the truss. They placed a shackle on top of the metal beam to position themselves for rigging. The shackle off the beam and fell approximately four feet, striking the employee on the back of right index finger.
Employee was lifting a desk with other employees into the back of a pick-up truck. When the employee bent down to lift and stood up, EE felt pain in EE's back and down EE's right leg.
Employee was attempting to open the back door of a UTV, and the front seat passengers opened their door to assist. Employee was able to get their door open before front passenger exited the UTV and placed their hand on the door jam. During this time the front seat passenger closed their door and smashed employees' thumb in door.
While dismounting Dozer 01, the employee led with their left foot onto uneven ground. Upon contact, they experienced pain in the left distal quadriceps area.
Employee was shoveling carryback material from under a conveyor belt tail area and felt pain in lumbar spine.
Employee was adjusting lifting shackle and overhead crane hook during lifting operations. The employee's right index finger was pinched between lifting shackle and overhead crane hook. The employee suffered partial traumatic transphalangeal amputation of right index finger with nailbed involvement.
Claims that EE hit EE's right elbow on the rear corner of EE's truck and then felt a sharp pain in EE's right shoulder while hitting the side of EE's trailer with EE's rubber mallet. Next morning EE could not move EE's right arm without severe pain. Sent to Concentra and diagnosed with a shoulder strain and contusion to elbow and placed on modified duty.
Employee was walking down staircase when EE stepped on second to the last step, the stair broke resulting in EE's left foot going through the step and hurting EE's ankle. Employee seen and released to full work duties. Employee had follow-up and seen 10/16/2025 and went on restricted duty.
While exiting light vehicle on driver's side, employee stepped onto pointed edge rock, right ankle twisted resulting in subsequent fall to ground. Injury sustained was right ankle avulsion fracture distal fibula
Employee was taking apart a 3-inch hose and the camlock struck EE in the lower lip, causing a laceration.
Employee sustained a nondisplaced fracture of right thumb (Distal Phalanx) when employee was ascending the stairs to access the belt 2260CON001 and the safety bar came down and smashed the employees right thumb.
The employee was leaving Shovel 4 and 6 pit in 6th gear when EE hit a pothole in the road. The employee stated the truck bed raised up a little and came down, causing the seat to bottom out. The employee was seen by Occ Med on 9/23/25 and was released without restrictions. On 9/26/25, the employee was seen by Occ Med and was returned to work with restrictions.
The employee was fueling Haul Truck 11 with Diesel Fuel when the hose popped off and splashed the employee in the eyes, face and chest. The operator was wearing PPE during the time of fueling.
Employee stepped in the inlet side of the cell at the tank house. They fell forward and injured left wrist and right shoulder.
Haul Truck operator hit an unknown bump, a few minutes after began to feel pain in EE's back.
Rubber tire driver hit a few potholes and felt a slight pain on EE's chest and low back. When the driver stopped to check equipment, EE felt dizzy and could not stand.
The employee was in the cab of the radial stacker when the 2450-002 luffing system base failed at end of shift causing the stacker cab to drop to the right side. The operator in the cab, experienced some discomfort to the left leg after the luffing system failed.
Employee was transporting a part (Core) by forklift when the part fell off the pallet during transport. The employee smashed their left hand between the part and pallet while lifting the part back onto the pallet.
While employee was walking under EE's truck on the driver's side for an air valve check, the right foot slid out from underneath EE. This caused all weight to shift to EE's left side and resulted in a left ankle sprain.
Truck driver was walking toward line up room in parking area. EE rolled EE's right ankle which resulted in an avulsion fracture
Shovel Operator was exiting the cab when EE tripped and fell on the metal grating. EE has cuts to EE's eyelid, cheek, nose and lower lip. EE received two stitches to lower lip.
Employee was walking when they tripped and fell on metal grating walkway causing abrasion on right wrist
Employee was fusing 4-inch T's when the employee bent over and grabbed the handle of the fusion machine. The employee's lower back locked up. The employee stated pre-existing injury.
While disconnecting the rear hose attachment of the CCV003 (Vac Truck) to clear a plugged hose, the top part of the hose whipped back and hit the employee's front tooth, cracking the bottom part of the right front incisor (central) tooth.
Employees were lifting a piece of ductwork into place to align the flanges to mark the holes, the ductwork pipe shifted pinching employees' thumb between the flanges resulting in a Laceration/Avulsion.
Employee was tightening lugs on tire when employee set impact gun down and smashed left pinky finger between rim of tire and impact.
Our employee was gathering equipment at the jobsite and accidently tripped on a cable tray. As EE reached out to break EE's fall EE came in contact with a sharp piece of pipe where EE cut EE's right hand. Our employee was taken to an urgent care facility where EE received 5 stitches in EE's right hand.
Employee was removing panels from screen media with Air Tugger, when employee was holding Air Tugger chain with EE's left hand. Employees left hand became lodged in the Air Tugger Block causing laceration to EE's left hand.
While loading Haul Truck 36, a shovel operator trainee made contact with the back of HT36 with the dipper. After dumping the operator continued onto an additional run at Shovel 3. After another full load/dump cycle, the operator stated they felt neck and back pain.
While driving through SH3 pit, a haul truck trainee employee hit a rough spot in the road. Initially the employee stated they were ok but after another run, they told their mentor that they were experiencing back pain.
Haul truck dumped out and was traveling to the next assignment. As EE headed down the Power ramp, EE's truck began to slide due to the roads being slick from the weather event just before the shift. Driver tried to correct the slide, and EE made contact with the berm with the right front corner.
Employee was lifting a 300lb propane gun with two other employees when EE felt a pain in the shoulder. EE worked through the remainder of the shift thinking the pain would subside, and when it hadn't, EE reported the incident to supervisor at the start of the shift the next day.
While walking near a running conveyor belt on a Copper heap leach system with PPE on (Safety Glasses) the wind gusted and dust entered employees head and face causing a dust particle to enter their eye. They rubbed their eye before seeking eye wash. Employee was taken to the clinic. The exam showed no dirt in eye but a small scratch.
Employee was driving Lube Truck 09 to Lone Star Stemming Plant to service equipment when the rear brake caught fire. While the employee was crouched down trying to put out the brake fire with the fire extinguisher, the right rear inner tire blew out. The employee was taken to the ER, evaluated and released.
Stood from crouched position, caused pain in knee. Employee is not able to walk or stand normally.
Truck Driver Trainee was putting on EE's coat as EE stepped out of the water truck cab. EE fell backwards and struck the handrail with EE's left rib area. EE was taken to the emergency room and got x-rays with negative findings on 12/6/24. The Trainee was x-rayed again on 12/10/24 and four non-displaced rib fractures on EE's left side were found.
Employee was stripping ground cable with a pocketknife. Employee was not wearing gloves at time of the incident.
Employee was entering a trailer porter-john when the trailer tipped towards them. Employee fell backwards onto loose rock, landing on knees and wrists. Employee sustained left knee contusion and right wrist strain.
Employee was rolling up the fuel truck hose after servicing a dozer at the 4650 dump in Lone Star pit. Their left leg gave away.
A front-end loader trainee hit the haul truck bed rail with the hoist while loading. This caused the haul truck driver to be shaken up and have neck and back pain.
Employee was operating a haul truck (HT32) and dumped the truck at the 4650 dump in the Lone Star pit. As employee left the area, the employee stated employee hit a bump or soft spot which caused employee to have pain in lower back.
While reversing EE's Haul Truck to park in a chock ditch, EE's rear tires dropped into the ditch causing EE's seat back to fail and when EE's body fell back with the seat, EE's feet slipped off the foot pedals and EE's right foot got caught between the brake and accelerator pedal causing pain.
Employee was stripping area on PH1 ROM Dump and twisted ankle while EE was performing the daily task of stripping area.
Employee received first bucket from shovel 2 and noted to the operator and Dispatch/Supervisor that the load was rough. They stated at the time they were OK, needed no medical attention, and incident was initially classified as first aid. On 10/24 upon follow up with Occ Med Provider, employee received restrictions.
Employee aggravated a pre-existing lumbar injury when shoveling thick mud from under the 2260 Tripper Car that is located at the Secondary Plant.
Employee was walking in parking lot to the line out meeting. Employee stated was trying to avoid a rattlesnake in the parking lot and rolled left ankle. This resulted in an ankle sprain.
Employee was grinding overhead, when they removed their face shield, they felt eye irritation, foreign body was discovered that required removal techniques beyond first aid.
The employee was walking to EE's vehicle at the end of EE's shift when EE stepped on uneven ground causing EE to trip and fall. Employee was complaining of serve pain in EE's right elbow, shoulder, and ankle.
Employee was stepping down from the Lube Truck and felt an uncomfortable sensation in EE's right calf. EE then had swelling and reduced range of motion in EE's calf. Upon follow up with occ med provider, employee was provided a rigid brace and restricted from climbing ladders and stairs.
Employee placed on work restrictions related to an increasingly worsening skin rash on the top of the employee's feet and shins, causing pain. Employee began first noticing rash about two weeks prior to informing supervisor, was evaluated by occ med with rash monitored, and subsequently given work restrictions by occ Med provider on 9/1/2024.
Mechanic stepped down from the rubber tire dozer to ground and felt pain in right knee. Seen by occ med physician and released for full work. Two weeks later 7/17 an MRI was conducted for which a follow up occ med appt occurred 7/22, where the employee was ordered physical therapy. At no point to date has the employee missed a work shift or been restricted from full duty.
While performing a PM on Haul Truck 38, the employee was removing a sump screen when EE felt a "pop" in their left shoulder. EE stopped work and contacted the supervisor.
Haul truck 66 was traveling behind haul truck 25 when the right-side rock guard of haul truck 66 made contact with the dovetail of haul truck 25 causing haul truck 66 to fishtail in a circle, jarring the operator from side to side inside the cab, causing injury.
Employee was loosening valve, Strained back and received restrictions.
At the beginning of shift the operator noticed the dome light cover was off and could tell the dome light was burned out. The operator applied slight pressure to remove the old bulb when the bulb shattered, cutting right middle finger. Operator was assessed by Rescue and taken to the clinic by Safety where the operator received 5 stitches.
Employee was mixing chemicals in the SX Lab, when they got (ammonium biflouride) granules in their right eye. Employee was wearing their safety goggles at the time of incident.
Operator experienced coughing and burning sensation in their lungs due to the presence of dust in the cab of Haul Truck 53.
Haul Truck 43 was being loaded on the left side of Shovel 2 and had two or three dippers already loaded in the haul truck. Shovel 2 was going to load another dipper full when the operator swung left and made contact with the left corner of the dovetail causing haul truck 43 to pick up and move three to four feet, jarring the haul truck operator inside the cab.
Climbed out of the front loader frame, noticed knee popped out of place.
While descending the stairs of the 2390 Tripper Car, the employees heel caught the edge of the last step and caused them to slip and fall forward onto their side and injured their left ribs and shoulder.
Employee was walking and stepped wrong, when trying to avoid falling they rolled ankle.
Employee was walking down the stairs at the stripping machine to the west bay and overstepped, causing them to slip, fall and hurt their left leg and right arm.

Employee was lifting guard to align keeper pins when it shifted and pinched EE's finger between the guard and shank.
12/19/23 employee was reaching back to adjust their seatbelt when leaving the dump and their hand hit the stem, causing the ear plug to jam into the left ear. 12/26/23 employee was seen by occupational medicine for follow up and given restrictions.
Working in the EW tank house pulling shorts, when EE began to feel EE's face and neck burn and itch.
Putting the product hoses away after delivering red diesel, the employee felt a pinch in their lower back. Resalting in the driver going to a chiropractor following the injury.
While on a red alert driver was outside cab putting on acid suit, when a large metal sign (6'x4') was blown over striking EE in the back. Driver was knocked to ground.
During the operator's break at Rader Tie down, the employee stepped out to clean the mirrors on HT44 and the door closed on their left hand, smashing their index and middle finger. This incident resulted in fractures to both fingers.
EE was scraping grease from the high-voltage rings of a shovel. EE rolled onto their belly to descend the ladder, and in the process, their legs and body fell from the opening/ladder, EE grabbed onto the ring to keep themselves from falling and loaded their shoulder at an awkward angle with their body weight. EE immediately experienced a burning pain and limited range of motion.
While a rebar wall was being lifted by the crane, the wall pieces started to fall apart and a piece of rebar fell and struck our employee on the arm and another piece fell and hit EE's hard hat. EE ended up being taken to the hospital.
As the employee was exiting their work vehicle, they stepped into a rut in the road and twisted their left knee.
Employee was walking from the parking lot to the lunchroom and rolled EE's right ankle on a rock just before the access into the Hydro met office building.
Employee was trying to free a stuck rigging hook on shovel dipper door. Employee used a sledge to try and free the hook. Using both hands to swing the sledge, employee freed the hook which in turn swung back and struck the employee's right thumb.
Helper was in the process of cutting the bands off core boxes. Helper put the knife back in a pocket without closing it. Helper reached back into the pocket for the knife, in doing so, cut middle finger on left hand. *Note: this was just a Medical Treatment case, with no lost days or away from work.
Employee was working in the EW Tank House assisting the crane operator on 08/21/23, when employee was exposed to acid mist. On 08/22/23, the employee reported to supervisor that the employee was experiencing swelling in both eyes, blurry vision, and discharge.
Employee hyperextended their foot when stepping down a step of the 2430CON002 Horizontal Feed Conveyor. Misjudged depth of step and rolled ankle.
After doing clean up with a shovel in the COS Tunnel on 8/12/23, the employee felt pain in their back. EE did not report it to the supervisor. On 8/14/23, EE reported this to the new area supervisor. The employee was then scheduled to see the provider and received restrictions.
The filter access door on the front of the haul truck cab was popped open. The employee did not notice the door was opened onto the walkway path of the Haul Truck and hit right knee on the corner of the door. This resulted in laceration and stitches.
Employee was performing their daily task when they felt sick and overheated. The employee was administered fluids both through IV and oral on-site and was instructed to stay in a cool area and continue to drink water.
WHILE STRIPPING 4/0 INSULATION THE KNIFE SLIPPED SIDEWAYS OFF THE END OF 4/0 ELECTRIC CABLE (WHERE THE LUG GOES) AND CUT THE LEFT PALM BELOW THE THUMB. EMPLOYEE WAS WEARING CUT GLOVES. EMPLOYEE REPORTED TO AMES SAFETY AND WAS TAKEN TO FMI RESCUE FOR EVALUATION. EMPLOYEE'S SUPERVISOR TOOK EMPLOYEE TO A CLINIC FOR FOLLOW UP CARE.
Employee reported a headache of increasing intensity, progressively feeling worse as the day went on, while still drinking water. Employee was transported to the hospital with signs of dehydration and heat exhaustion.
While descending the ladder of Haul Truck 22, the employee slipped off the fourth step and the right ankle was hung up on the rail of the ladder and twisted as the employee fell forward.
Drill Helper was hit on the upper lip, causing a laceration about 2 cm long and knocking the left front tooth loose as the Drill Helper was attempting to empty the last remaining section of core from the tube.
While the employee was positioning the wheelbarrow for a co-worker to start loading material, their finger got caught between the structure of 2140CON001 and the handle of the wheelbarrow. Resulted in a fracture to the tip of ring finger.
Employee was getting back in the cab when felt their knee pop as they stepped down into the cab of Water Truck 24. The employee reported swelling and pain.
Employee was walking around Drill 9 when they tripped and fell and hurt the right ankle and scratched the right hand. The employee was evaluated by Site Rescue. The incident resulted in a right ankle fracture.
As the employee lifted the drill steel with the drills jib the steel got caught on the drill head, causing the bottom of the drill steel to kick towards the drill rig. When the drill steel kicked towards the drill the employee put EE's arms out to redirect the drill steel into place and from hitting the drill. The pipe hit EE's forearm against the metal frame of the safety gate.
Employee was closing the dividing wall in a conference room. They attempted to push it an additional 4 inches to the rubber bumper. At this point the employee kneeled to push against the wall. They then felt a sharp sting and pain in their right foot after pushing the wall. Employee must wear a Cam boot as result of incident.
Employee was walking up the staircase of the Screening Tower when they did not gain proper footing on the final step, fell and hit their right knee causing discomfort.
EE was unloading Sulphur and got the supplied breathable airline caught on the heel of their boot when stepping backward. This caused EE to trip and fall on their backside. While trying to brace from the fall with hands EE hairline fractured a bone in left wrist.
Employee was replacing a coolant hose on a haul truck. EE leaned back to reach a hose. EE used EE's left arm to pick self up. As EE was pulling self up, EE felt EE's shoulder pop out of place. EE's shoulder was reset at the hospital, and EE returned to work.
Employee was disconnecting the fuel hose from the haul truck. When the employee turned to replace the hose, employee stepped in a hole and sprained the left ankle.
TECH WAS REMOVING BUCKET TEETH AND TOOTH WAS STUCK. TECH USED HAMMER AND HIT THE TOOTH. A PIECE OF FLY METAL BROKE LOOSE AND HIT TECH IN THE LEFT KNEE. TECH FINISHED WORKING THE SHIFT.
Employee took off hardhat to look for a landing ring in the toolbox located under the right side of the CT20 drill rig. After retrieving the part employee backed out and stood up hitting head on the catwalk. First Aid was given and employee was taken to the Hospital and received two staples to the wound. Returned back to work with no restrictions.
Employee was tasked to collect samples from the sample port at the south end of the PLS Tank. The employee crossed over a 24 HDPE pipe to get to the sample port. After retrieving the sample, EE crossed back over the pipe, and felt a pop and pain in the left knee.
Employee was headed back to the pit after dumping out, when EE hit a pothole in the road which caused pain to be felt in the upper back.
Employee smashed their first digit of the right hand when EE was in the haul truck driver seat and reached over the buddy seat, pulling the passenger door closed.
While employee was performing their pre-shift walk around, they tripped over a handle for a cover on the haul truck deck and landed on their left side against the fire suppression system, bruising their ribs.
Employee was attempting to dismount Rubber Tire Dozer, when the employee tripped, fell on the deck and felt pain in left leg.
Employee received contusion of the left hand when pressing the release for the dump lever on their haul truck and felt left hand pain.
EE was putting away parts in the connex, as EE went to close the door, EE smashed right hand which resulted in the smashing of the right-hand middle finger. EE came inside and immediately reported the incident.
While employee was backing under shovel dipper to get loaded, the steering got tight and hard to turn, causing the operator to feel a pop in the left shoulder while trying to steer.
Haul Truck operator dumped out on waste dump when EE hit a pothole while exiting the dump causing EE to bounce in the seat. The operator felt a "jar" in their back.
While employee was performing their daily task installing a bracket on a repeater pole, they struck their left hand with a 4-pound sledgehammer, causing swelling.
The driver was bit by a wild coyote and is currently following rabies vaccination protocol.
While releasing the park brake, the switch came back and hit operator in the palm, causing pain to right hand and wrist.
While the employee was raising boarding ladder of the Haul Truck, the employee had back turned away from the ladder as it was being raised. Left wrist was caught in pinch point when ladder reached the full upright position.
The operator was dismounting the Haul Truck when EE missed a step descending the staircase of the truck. This resulted in an injury to the right knee.
When reaching back to set the parking brake lever on Haul Truck, the employee stated they felt pain and stinging in right arm. Employee received restrictions 11/4/22.
Employee was attempting to reposition cathode on strongback. Cathode fell smashing left middle finger between cathode and discharge chain severing the tip.
A boulder fell out of the dipper teeth into Haul Truck bed, causing a rough load, resulting in neck and shoulder pain.
While engaging the bank to get a dipper full of material the teeth encountered a hard bottom jarring the shovel. This caused the foot latch on the operator seat to release, and the operator seat to slide back and hit the stops. When the seat hit the stops the operator aggravated an old back injury.
Employee was using the handle of the hammer to hold the bolt's head in place while another employee was running the impact. The bolt spun and the employee's hand slipped, coming in contact with the spinning bolt, causing a laceration that required stitches.
Employee was trying to reel up a cable for the shot. Employee had to use the back step to fix the cable on the reel. When finished, EE stepped down and felt a pain in EE's right ankle.
Employee was signaling crane to lower load. The load was being placed in front of crane and light plant was in EE's line of sight. Injured employee bent over to place cribbing on the ground. The load was stopped by the operator but shifted, striking the employee in the side of the head. This caused a laceration, requiring 8 stitches.
Due to a BO valve that over rotated past the open and close "stops" created a strike point between the valve and the upright pipeline support for the fuel line. The employee's middle right finger was smashed, resulting in a fracture.
Employee operating an extended reach forklift was pushing the Point Sheave into the bore that accommodates the weldment lock. An employee was holding the weldment lock plate. The fork slid off and pinched the top of the employee's fingers between the shaft weldment lock plate lock and the fork. The employee suffered an injury to the tip of #4 digit of the left hand.
The employee tripped on the stairs coming from cab of Shovel 2 down to the house of the shovel, injuring the left knee.
The employee was rolling up hose reel 10wt on lube truck 09. When the hose got to the end of rolling up, it spun out of EE's hand control and struck the right side of EE's face. The coupling on the end of the hose hit the employee's cheek bone area of the face. The employee was wearing safety glasses.
Employee was torqueing lug nuts on right front wheel of LD02, using a pneumatic torque gun. While positioning the torque gun and socket on a lug nut, EE did not move hand before pulling the trigger, smashing EE's right hand between the reaction arm of the torque gun and the loader wheel.
Employee was working on the PH cleaning of the reverse osmosis (RO) system but did not notice that the temperature was rising. EE went to take a break in the work truck AC. While taking the break, EE drank some cold water which immediately made EE shiver and get sick. EE's supervisor shortly arrived at the RO after the employee got sick.
Employee was a passenger in a light vehicle on July 13,2022 when a man van failed to stop in time approaching a stop sign and rear ended the light vehicle causing lower back soreness. Restricted Duty implemented on December 22, 2022 due to increased/unresolved pain and MRI results.
Employee was walking back to the Polaris when employee stepped over a berm and twisted right ankle. The employee continued to walk towards the Polaris and twisted the ankle a second time.
Employee (Employee 1) was working at the hoist drum with another employee (Employee 2) during hoist rope change. Employee 2 was running the hoist tuggers and Employee1 was working to guide the end of the hoist ropes into the holders on the hoist drum. While Lifting and pushing the end of the hoist rope into place, there was a pop sound from Employee 1 right arm.
Employee rounded a 90 degree corner on personal motorcycle and lost control, skidding along pavement on 5/26/22. Employee was evaluated at the ER 5/26 and Occ Med 5/27. Upon 4th follow-up appointment with Occ Med 6/6/22, employee work status changed to restricted.
Employee was walking down the stairs of Haul Truck, when EE felt a pop in the left knee.
Employee was sitting in a chair, when they bent over to pull up white coveralls and felt sharp pain in back.
Driller was attempting to slide the casing up the rod slide from the rod table and lost control of the casing when EE#'s right hand slipped off, causing it to drop down the slide unexpectedly. This resulted in the individuals left ring finger being caught between the casing and rods on the rod table. Injury resulted in amputation of the ring finger.
Employee was collecting samples on a drill pattern, when EE stepped over a track left by the drill and lost EE's footing on loose material, causing EE to fall and injure EE's right knee.
Employee was walking down stairs of Haul Truck 35, when EE's knee gave out. Felt pain in the left knee and was unable to put pressure on it.
The driller was using a hex tool wrench to remove a 2" pipe fitting from the side inlet body. While pushing hard to break the fitting, the wrench slipped and EE's hand struck the drill mast. Wrong tool for the job was used. Driller had used a Hex tool wrench in place of a Pipe Wrench. Pipe wrenches ranging in size from 18 to 36" were already on site. Incorrect Safety Glove worn.
While operator was changing between equipment, the employee's right eye began to feel irritated. EE was seen by Rescue and sent to ER for further evaluation. The operator received prescription treatment.
When employee was getting back into their equipment (Dozer 14), the tip of the right index finger was smashed in the cab door, resulting in stitches.
Individual was working on laying out 30 foot sections of drill pipe onto the pipe handling table. Individual caught EE's hand between the drill pipe and fixed drill rod slide, that resulted in a fracture to the index and middle finger (no lacerations).
Employee complained of neck and back pain after receiving the first dipper of material from Shovel 6.
Employee was getting loaded at Shovel 6 with left arm resting on the window. Resulting in shoulder pain.
Employee was stepping into man van at beginning of shift when employee felt knee pop. Upon exiting the van, employee felt pain and was unable to put weight on left leg.
An operator and trainee on Haul Truck 22 were rough loaded at Shovel 4. The operator proceeded to dump load, tied down and contacted supervisor to report event. This resulted in the operator to experience lower back pain.
Miner was unloading the top piece of a 2 piece bypass wedge from the bed of a pickup. In doing so the wedge slid down the plastic protective cover of tailgate, and miner tried to catch the piece from falling which resulted in left hand caught between wedge and the ground. Causing a fracture to the left ring finger and lacerations to two fingers.
Employee started experiencing back pain after the man van hit a pothole on the haul road coming in at shift change the previous day.
Employee lost footing going down the stairs of Shovel 4. Employee slipped and tumbled to the ground and suffered impacts to EE's left rib cage, back, and one knee. Employee was transported to ER, X-rays and scans conducted showed no evidence of fractures.
Employee cut their middle finger on a sheet of copper while attempting to remove an imbedded pre-strip clip from a bundle with a crowbar. The laceration required two stitches.
Employee was exiting the Man Van at the Crusher Road Tiedown when employee rolled right ankle.
While attempting to turn at the Crusher pocket, the employee noticed the steering was tight on Haul Truck 27 and continued making the turn. This resulted in the employee feeling pain in their left shoulder.
The operator was bending down to pick up a drill pad that needed adjusted prior to unloading Drill 15 off of the Tow Boy. As EE bent down, EE felt a pop in EE's knee, and EE fell to the ground. This incident caused dislocation to the left knee.
Employee missed a step and slipped while attempting to exit the cab of Water Truck 17. Employee fell to the ground hitting their left foot. Employee then climbed back into Water Truck and moved it to the side and called dispatch. This incident resulted in a fracture to the left ankle.
Employee was assigned to shovel and pull drip. At the end of shift the employee noticed tightness in EE's right arm. When employee arrived home, the employee noticed swelling in the right forearm area. Employee went to the ER and was told that the swelling was from a possible busted blood vessel from the over use of the arm.
Employee was cleaning Shovel 6 pit area when employee struck a rock, jarring employee and causing lower back pain.
While employee was exiting the Lube Truck, they missed the step on the ladder and strained left muscle.
Employee was pushing cathode sheet to the discard bin when EE felt a tightness in lower back.
Employee was finishing a break when the toe of EE's boot got hung up on the seat of the bench causing EE to fall on EE's left knee. The fall caused a small laceration.
The operator of Haul Truck 38 was traveling empty off of the 4675 waste dump when EE hit a pothole, jarring EE causing pain to EE's lower back.
Employee was cleaning out radiators at the secondary crushing plant when employee started experiencing heat exhaustion.
While working on Drill 14, the employee was working out of a man basket attempting to remove a pin from a cylinder. The hammer got caught on the safety cable for the JLG controls causing the magnetic base to become loose and strike the employee in the left eye. This caused a small laceration.
Employee was removing weeds when EE felt pain in EE's left shoulder after putting pressure to up root a bush with the shovel.
Employee received friction burns by a rope on the interior of left hand when employee grabbed a moving rope to slow the shovel cable lowering from the pole bridges at Shovel 1.
Employee was conducting maintenance when organic solution splashed on the right leg of EE's PPE. Employee immediately washed off the solution and changed. 5/28, the employee notified the supervisor that the skin was irritated and a rash had developed. Employee was seen that day by the occ med physician who treated the rash and instructed to keep dry and clean until healed.
While two employees were installing an A/C belt on Haul Truck 25, the a/c compressor fell and hit the employee in the back between the shoulder blades. This resulted in a back contusion.
Employee stated that EE was moving concrete with a wheelbarrow and strained when EE was pouring out the concrete causing an umbilical hernia.
Employee rolled their right ankle stepping down from the inbound gate scale, resulting in a fracture.
Employee was driving home in their personal vehicle after working the second shift of the rotation, when EE claims EE blacked out at the wheel.
Employee smashed finger while handling a snubber pin. The mechanic threw the 50lb pin onto the dipper and it started to roll back. The mechanic tried to catch the pin but as it fell and hit the service truck bed, it smashed finger. The incident resulted in a fractured right index finger.
Employee was backing up the Haul Truck to be loaded at the Shovel. The employee turned to roll down the window to hear the horn of the Shovel. While the body was still turned, the shovel operator started loading the Haul Truck causing the employee to feel pain in back.
While employee was closing the window to a 994 Loader the employee's finger was caught between the window handle and a mounting bracket for a dispatch screen.
While employee was exiting the cab onto the drill deck, the operator felt a pop in right foot.
Employee was pulling a twisted power cable for the #4 shovel on the 4400 level. Employee had both hands on the hot stick, when the cable twisted two of the employee's right fingers came off the handle causing pain.
Employee was rough loaded while operating a Haul Truck. Shovel lost power causing dipper to drop and make contact with Haul Truck bed.
Employee was working at Lone Star leaching pad, cell 9 mod D. The employee attempted to cross a 12" and 4" pipe that was butted together by stepping over. EE stepped on 4" pipe to get over 12" pipe causing EE to slip and sprain EE's left knee.
Employee was removing material from behind the kingpin on the horizontal portable when material sluffed down hitting employee's legs, knocking employee down. Employee hit right lower back on the structure behind employee.
Two employees were attempting to lift a crusher lube pump from the back of a pickup. The employee lost grip on the pump due to oil on the pump and EE's gloves. This caused the pump to fall and smash the employee's finger between the coupling of the pump and the tailgate of the pump. The tip of finger was fractured and two stitches were provided to close the laceration.
When employee was stepping over a pile that they were in the process of cleaning, EE stepped on a rock causing left ankle to roll.
While attempting to hook up a Grinch Winch on the back of a Polaris Ranger, the employee experienced pain in the lower back.
Employee strained lower back while throwing a bag of ice onto the drill deck.
According to an independent medical review and MRI imaging received 10/6/20, the operator returned to work too early from a personal surgical repair, with the movement of the motor grader controls on 9/3/20 aggravating the shoulder and causing the need for a new surgery.
While employee was reeling in hose from water truck, the employee smashed the tip of EE's right thumb between the hose reel handle and the hose locking mechanism, resulting in re-embedding of the nail.
While employee was operating a grader, the employee got what EE believed to be dust in EE's eye.

A water truck driver twisted EE's left ankle while dismounting EE's water truck on the last night of EE's five scheduled night shifts. Employee reported to work on 7/31 after four scheduled days off, worked half of the shift, then reported to supervision that EE's ankle pain had worsened attributed to the original twisted ankle event.
While employee was reeling in a water hose at the back of a water truck. A metal containment box lid fell and smashed the employee's left index finger resulting in stitches.
While employee was getting into the cab of Water Truck 2 the wind caught the driver side door and slammed it shut while the employee's hand was in the door jamb, causing a laceration that required stitches.

Employee was dismantling an old screen using a pry bar. While attempting to loosen a lower piece of angle iron, another piece of angle iron fell from higher section of the screen, striking employee on left wrist.
Shovel Mechanic was removing a wedge from the tooth adapter using an impact gun when the bit got stuck inside the wedge. In the attempt to remove the bit from the wedge, the mechanic suffered a smashed finger. As mechanic sat there holding the hand, mechanic started to feel faint & ultimately fainted, falling to the ground from seated position causing laceration to chin.
Operator was unplugging a feeder using mental bar when the bar slipped and pinched EE's right pinky finger causing a laceration and fracture.
While employee was closing a rolling gate, their left ring finger was caught between a roller and the roller guide, amputating the tip of the employee's ring finger.
While operator was performing pre-operational inspection of haul truck, the employee rolled the ankle walking on the ground around the haul truck.
Employee was installing a ground insulator onto a pole at the lineman yard (Clifton Training Center) when tightening a nut with an impact wrench, employee's left pointer finger was cut by a cotter key. Employee was wearing leather gloves that were torn during the incident.
