It appears that the employee was loosening a hose on a piece of equipment; the wrench slipped and hit him on a tooth. To date the tooth has not been repaired and no work missed due to the incident.

Employee was using come a longs to unload shuttle car tire off mantrip. He was pulling on the come a longs when they got wedged, he was standing in mud. When he pulled, the come a longs broke loose causing him to twist his knee.

A re-write of the incident indicated that the employee was jacking a rail supply car on the track and the edge of the car struck the rib area. CT scan in April indicate old rib fracture. Comp.denied claim. Received similar medical treatment/off 48 days from Jan. 2012 through June 2012 (prior to incident). Continue to investigate claim as to work related.

Employee was prying with slate bar, slate bar slipped, causing employee to fall forward putting his body weight on his upper arm, employee did not report accident to magmt. until 5-21-12, employee was seen by a physcian for muscle strain.

Employee was using a metal punch to score a notch for drilling through a piece of metal, when he hit the punch a piece of metal came off embedding in left hand

Employee was attempting to remove an oil pan plug using pliers, the pliers slipped and his little finger on right hand hit the oil pan fracturing it. Employee was seen by a physician and had surgery performed and pin placed in finger.

Using a hammer and punch to knock pin out of boom of miner, a piece of metal either came off of hammer or punch, becoming lodged in the right side of neck. Employee was seen at ER, then adivsed to have it surgically removed. Employee returned to work the next shift, metal was removed on 1-9-12, returned to work on 1-12-12.

Employee stated that while using pick hammer to punch hole in oil can, he struck his thumb on left hand. Employee was seen at ER where physicians lanced nail bed to drain blood. Report is 255 days late as per a part 50 audit.

Come-a-long broke loose from tailpiece and hit employee in low back toward the left hip. Employee continued to work until 6-3-08. Missed work 6-3;6-4;6-5. Did not know he was off due to the injury until 6-6-08.

EE was assisting miners in making belt splice at 3-head.EE was pounding nails into splice w/ hammer,hammer struck his left thumb.EE sustained dislocated thumb and needed 2 sutures.EE was released to RTW on 1/16/08.EE worked part of his shift on 1/16/08.EE went back to doctor and is not scheduled to RTW until 1/22/08.Miner proficency and awareness were factors in incident.

Employee was assisting setting conveyor belt tail piece. Employees tried to pry loose the stuck belt, the come-a-longs holding the tail piece loosened and the tail piece fell down on the employee's right foot. Compliance with rules/regs not a factor. Protective equipment not a factor. Mining equipment/sytems not a factor. Miner proficency a factor.
