Michigan Paving and Materials Company · Caught in, under or between a moving and a stationary object

Employee was removing spray bar clamps from an old wash screen. After removing the hardware, the U-bolt was stuck in position around the bar, Employee used a 10" pry bar to free the clamp. While pulling downward the pry bar slipped and pinched the palm side tip of EE's left ring finger, causing a laceration requiring two stitches.

Employee was using a razor knife to cut a piece of rubber for a conveyor back plate. The knife caught the edge of a steel plate, used for a template, and skipped up causing the employee to pull the knife across the right thigh, cut through the jeans and cutting the employee. The cut was about two inches long requiring six sutures.

While cutting out a section of damaged conveyor, approximately 8" x 10", to insert a patch the knife slipped out of the conveyor causing employee to stab them-self in the thigh resulting in a laceration requiring 5 staples to close.

While trying to remove a bolt with a wrench from a dragline crane engine, hand slipped with the wrench. Employee hit pinky knuckle on right hand and caused an avulsion fracture.

Employee was cutting piece of rubber when the blade came out of the folding utility knife EE was using causing a laceration to EE's right index finger. Employee received 4 sutures. Employee was wearing PPE, hard hat, safety glasses and gloves.

Employee was removing a 5-6' diameter 3000 lb drive sheave on a horizontal impact crusher to prepare rotor for removal. Sheave was seized on shaft and popped off once it came loose.

Employee was prying apart 2 pieces of plate to add a spacer to be able to cut the top piece. While prying the pry bar slipped out from in between the two pieces and struck the employee in the lower mouth causing the inside of his lip to be cut by his teeth.
