Employee was cutting piece of rubber when the blade came out of the folding utility knife EE was using causing a laceration to EE's right index finger. Employee received 4 sutures. Employee was wearing PPE, hard hat, safety glasses and gloves.

EE WAS MAINTAINING A CONVEYOR WHEN HE LOST HIS BALANCE & FELL TO THE GROUND STRIKING HIS RIGHT WRIST ON A ROLLER FRAME

EMPLOYEE WAS OPERATING LOADER WHEN LOWER BACK STARTED HURTING HIM.
