While installing a sweat on hub on the loadout belt conveyor drive, the hub stopped moving on the gearbox shaft 1/8" shy of its desired location. The victim was struck by a glancing blow of a 4 lb. hammer in the mouth. The hammer struck the hub before striking victim in mouth.

Showing all 3Employee was prying a form loose. A slate bar slipped and hit employee in the nose, requiring two (2) stitches.
Employee was beating a rock out of scalping screen using a sledge hammer. While hitting the rock, his right hand on the handle of the sledge came in contact with another rock. He did not seek medical attention until November 17th. He was told he had a fractured finger on the right hand following an x-ray. No other medical attention was advised. Employee did not miss any work.
