A piece, of rock fell across the miner. While, ee was removing the rock with a slate bar, the rock broke in half, and the slate bar slipped striking ee in the nose causing a laceration. After finishing the shift ee went to the Doctor, and received butterfly stitches.

Working the ratchet on a chain hoist when ratchet broke and the chain wrapped around his arm NOTE: EE worked did not file the inital incident report with the company until 8/12/2010. EE continued to work until November 16, 2010 at which time he went to see his doctor who restricted him from working.
