Showing all 15Was removing a plate from the west drag when it slipped from a pry bar that was holding it up and pinched the employees left index finger against a washer of the long bolt EE was taking out, laceration on the front side first and second sections - 4 sutures and splint.
Forehead laceration
Production Attendant using a sledge hammer to knock in finish mill doors when the hammer bounced and struck the employee in the face. Resulting in a laceration to the eye lid requiring sutures.
Using hammer to move railcar offloader door and struck finger causing fracture and avulsion.
Chain fall was hooked to liner that had pad eye welded to it. The pad eye broke loose and liner fell and hit EE in the right thigh. EE received a laceration on right thigh.
Employee had some cement dust get into left boot causing a cement burn to the outside of left ankle. Medical treatment was given and placed on restricted duty. The employee reported the incident to supervisor on 9/17/2016.
Employee swinging hammer and struck left index finger resulting in lacerations requiring sutures.
Employee was in the process of opening a F/M separator door. When he was pulling down on the wrench he felt a pull in his right shoulder.
Employee was doing a PM of the FM separator. He noticed some build up of material that was not allowing the dampers to close properly. He then went to knock the build up off with a hammer and while doing so some material fell into his left eye. We took him to the clinic and the doctor reported a scratch on his cornea. He was given an antibiotic cream. No restrictions were given.
The miner was in the process of rebuilding a 300mm pump assembly. While holding a wrench in place the wrench slipped and his right pinky finger was pinched between the wrench and the flapper arm on the pump. There laceration required four stitches.
Employee was removing a strut pin from a Cat 773B Haul truck. He used a sledge hammer to strike a pin driver and the pin driver chipped on the end. The chip hit his left forehead. He was taken to the hospital and the chip was removed and received stitches,
Injured employee was assisting another EE to switch a valve on top of the overtrack silos. He put his hand on the end of the bar and helped push up. When the bar moved up he got the little finger on his right hand caught between the bar and the overhead old FK line causing a laceration requiring 4 stitches.
Employee A hit the end of a chisel with the hammer and a small piece of the chisel fractured off upon impact. The small piece hit Employee A in the left arm. The wound quickly stopped bleeding and he was taken to Urgent care. At this point it was not reportable. The employee had a follow up and fragment was removed and wound stitched.
Employee was cutting a thick rubber hose with a pocket knife. The knife caught a metal stiffener on the inside of the hose. The locking mechanism faulted and the blade came back and cut his finger. The employee went to the doctor and received 5 stitches. He returned to work with the doctor giving him restrictions based on his pain tolerance.
