The injured employee and co-working were removing a metal canopy from a roof bolting machine. As the employees began to lift the canopy with a chain and ratchet to re-position the canopy, the chain and canopy slipped, allowing the canopy to shift, which resulted in an injury to four fingers on the left hand. (No broken bones). Cut glued together.

EE was using a large hammer to hit on a belt roller to break it loose from the steel structure. EE was holding the roller with their left hand. As EE was swinging the hammer with the right hand and hitting the roller, they missed the belt roller and hit a finger with the hammer. This resulted in a laceration to the left ring finger, requiring six (6) sutures to close the wound.

The injured employee was attempting to remove a cord lash or nylon band from a bundle of roof bolts. The employee was using a utility or razor knife to cut the band on the bundle of bolts. As the employee used left hand to cut the banding material, the knife slipped and cut right index finger.

The employee was using a rock bar to pry on a bundle of metal straps to get to a bolt on the back of the roof bolting machine, when the bar slipped causing the employees right arm to come in contact with the corner of the metal straps resulting in a laceration to the right wrist when required (5) stitches or sutures.

The injured employee and Co-worker was removing the slack from the conveyor chain of a continuous miner. The employee had removed links from the conveyor chain and was re-assembling the pins and keepers in the chain. A sledge hammer was being used to drive the lock rings onto the pins when the co-worker hit the injured employee's left little finger with the sledge hammer.
