Employee was removing large stones from the hazemag impact crusher (a blockage occurred in the discharge area). The employee was standing above the rotor and off to the side to get a good angle with a 6ft pry bar to dislodge the material, when a stone approximately 4'x 2' fell out of the chute and hit the rotor causing the stone to bounce up and strike the employee's left shin.

Showing all 3While adjusting shims on crusher pry bar slipped off and struck employee in the cheek causing a cut. Laceration to right cheek, needed eight sutures to close wound.
Employee found an issue with a power cable splice on SC65 screw, he was using a retractable safety knife to cut the outer electrical jacket off the splice when the knife came down cutting him below the left thumb. It took seven stitches to close the wound.
