Employee was removing large stones from the hazemag impact crusher (a blockage occurred in the discharge area). The employee was standing above the rotor and off to the side to get a good angle with a 6ft pry bar to dislodge the material, when a stone approximately 4'x 2' fell out of the chute and hit the rotor causing the stone to bounce up and strike the employee's left shin.
While adjusting shims on crusher pry bar slipped off and struck employee in the cheek causing a cut. Laceration to right cheek, needed eight sutures to close wound.
Employee had just finished loosening the tail pulley. When he moved from the sitting position to standing up, he felt a slight pop in his knee, spraining his left knee.
He stepped off the back of Michigan CAT's lube truck while facing forward and missed the blocking on the ground that was used for steps and stepped down 2 feet twisting his left knee causing a strain to his knee.
Employee found an issue with a power cable splice on SC65 screw, he was using a retractable safety knife to cut the outer electrical jacket off the splice when the knife came down cutting him below the left thumb. It took seven stitches to close the wound.
Employees where changing screens on a 7 X 20 screen. When they were lifting the screen into position to slide into the second deck, employee felt sharp pain in his lower back.
Employee was welding above his head, a piece of slag from welding popped off and got into employee's left eye
Drill steel that sits in holder became loose while setting up at new hole. As employee began drilling new hole, drill steel fell out of its holder, employee tried to catch the drill steel. Employee felt pain in his left arm just above the elbow.
Employee was fueling unit #143 (Picker). When employee stepped down off of track, employee's left ankle rolled on slippery surface, causing strain.
Employee was doing general maintenance on clam. Employee walked back to picker to get tool. Rock a few feet away tipped over onto leg.
Employee was walking across haul truck from engine compartment to rear frame platform. His feet slipped out from under him causing him to fall landing on metal beam.
Employee was preparing to put oil in his equipment at the start of his shift. He decided to turn on the headlights because it was still dark. After coming down off of the piece of equipment he took a few steps and stepped onto a rock, turning his ankle.
Employee was grinding steel on #801 feeder plate and metal blew in between face and safety glasses. Dust got into right eye.
While opening the lip on the crusher the lid slipped out of his hand and struck his left arm.
Cleaning debris off the secondary crusher with an air chisel.
Splitting rocks with a jackhammer. The rock split in an unusual manner and rolled pinning the operator's left leg below the knee.
WHEN DOING SCREEN REPAIRS HE SLIPPED ON THE SCREEN SUPPORT SYSTEM AND TWISTED HIS ANKLE
THE OPERATOR WAS SORTING MATERIAL. WHILE LIFTING A LARGE ROCK, THE LOADER TILTED FORWARD AND THE ROCK SLIPPED OUT OF THE BUCKET. THE LOADER ROCKED BACKWARD AND THE OPERATOR HIT HIS HEAD ON THE CB RADIO KNOB.
GRINDER KICKED BACK AND STRUCK THE MINER IN THE KNEE.
WHILE LIFTING THE FEEDER MOTOR IN PLACE HE PULLED HIS STOMACH MUSCLES.
