Showing all 6Employee was using a utility knife to cut drip line on the leach pad. Employee was cutting towards his body and not away from his body. As the employee was cutting the drip line the utility knife slipped and lacerated his left forearm requiring eight stitches.
While using a razor knife to cut a section of conveyor belting, EE cut his left thumb. The cut required 5 stitches.
Miner was tightening bolts on a screen shaker with a wrench when the tool slipped off of a bolt causing his hand to strike the side of the shaker. Miner was taken to hospital on 5-11-12 but no fracture was found. He complained that his hand was still sore. Took him back to hospital on 6-12-12 and fracture was found.
Miner was scaling frozen mud from agglomerator when he was struck by a chunk falling from the top, glancing off of his hard hat and hitting his shoulder.
Employee was cutting plastic tubing with a razor knife and was cutting towards himself when the blade contacted his left forearm leaving a 1 1/2" cut.
