Caught in, under or between a moving and a stationary object

BAD BRAKES/BAD STEERING, STEEP GRADE 15%. EMPLOYEE WAS TEST DRIVING THE WATER WAGON ON THE ACCESS ROAD WHICH IS ON THE SOUTH SIDE OF HOMESTED PIT. EMPLOYEE MUST HAVE LOST HIS BRAKES, POPPED OUT OF GEAR, LOST STEERING & TRIED TO JUMP. HE WAS CRUSHED BY THE TIRE.

Operator was running a dozer on a side hill for a long period of time causing a pain in EE's lower back

Working on CV 17. When entering the HSI crusher EE slipped and fell, EE tried to catch self and hit EE's shoulder. Then was help by EE's coworkers by making sure EE was ok and called mine supervisor before transporting EE to main office.

A Process operator was using a 30lb digging bar to clear lodged material form the jaw crusher. As the operator was working to clear the lodged material, EE felt a pinch in the lower back.

A mechanic was outside the truck shop in the out of service area coning off a light duty truck. And while doing so lost EE's footing and fell on EE's shoulder.

Struck against stationary object

Miner was surveying the pit when he stepped on a rock spraining his ankle, Miner was initially diagnosed with an ankle sprain but returned to the doctor two weeks later and was re diagnosed with a foot fracture.

Employee was removing a digging bar from the travel way to secure a safe work platform when the bar slipped from his hands through the grating. When the employee reached for the bar to stop it from falling his right ring finger got caught between the railing and bar causing a laceration and requiring one stitch.

Employee was using a utility knife to cut drip line on the leach pad. Employee was cutting towards his body and not away from his body. As the employee was cutting the drip line the utility knife slipped and lacerated his left forearm requiring eight stitches.

An employee was removing cutting edges from the bucket of a IT-28 when one of the plow bolts began to spin cutting through his glove and lacerating his hand. The employee was taken to a medical facility where he received medical treatment for the laceration and was released to full duty status.

While using a razor knife to cut a section of conveyor belting, EE cut his left thumb. The cut required 5 stitches.

Struck by flying object

Employee was standing outside of the lab building on a windy evening, directing a forklift when she felt something enter her eye. Employee was wearing safety glasses at the time. Employee returned to work after hospital flushed out her eye and prescribed antibiotics.

Struck against stationary object

Miner was tightening bolts on a screen shaker with a wrench when the tool slipped off of a bolt causing his hand to strike the side of the shaker. Miner was taken to hospital on 5-11-12 but no fracture was found. He complained that his hand was still sore. Took him back to hospital on 6-12-12 and fracture was found.

Mechanic was grinding on a discharge belt when a small piece of metal embedded itself in the miners right eye. Witness said the miner was wearing dual eye protection.

The miner & drill operator were changing the striker bar on a top hammer drill. After replacing the striker bar, they stepped away from the assembly at which time the drill operator lowered the drifter onto the head & exited the cab. The miner approached the assembly & proceeded to align the bolts. The assembly shifted causing his hands to slip up & pinch his left thumb.

While moving a heavy VSI crusher anvil onto a pallet, the employee got his hand between the old anvil and the replacement anvil, causing a laceration on his right middle finger that required stitches.

While lifting a tail pulley in place on a conveyor the employee suffered a strained shoulder when the tail pulley slipped and he tried to take the weight of the tail pulley.

Miner reported that he slipped on plastic liner, but that he was OK. Miner AWOL for 3 days before calling in to report that he was seeing a doctor on his own for a strained back. Safety Dept notified 12/09/2011.

Miner was scaling frozen mud from agglomerator when he was struck by a chunk falling from the top, glancing off of his hard hat and hitting his shoulder.

On 10/26/11 a Lab Tech was found in the lab storage room crying. He said he had chest pain & shortness of breath. Emt's on scene at 2:05 pm. ee is overweight, has high blood pressure and diabetes, heart attack was suspected. At 3:15 ee arrived at the hospital and was stable and responsive. On 10/27/11 ee called and said he was OK.

Employee was cutting plastic tubing with a razor knife and was cutting towards himself when the blade contacted his left forearm leaving a 1 1/2"" cut.

Employee got out of his pick-up truck, turned in the open doorway to retrieve his respirator and while doing so the wind blew the door shut, forcing the employee's left knee to impact the truck body.

Contact with electrical current

Driller was raising mast and struck an overhead power line. Driller Assistant was holding wireline at the time and received electrical shock.

Employee was attempting to enter a closed door to the maintenance shop with his hands full of parts. As he was opening the door he placed his foot on the edge of the cement pad in front of the door, lost his balance and strained his back.

Crew in process of breaking drill string. Drill rod was unthreaded from string. Employee grabbed drill rod, lost grip with right hand as rod was being lowered. Drill rod came in contact with left hand causing laceration to left index finger and fracture to bone above first joint.
