EE was attempting to cut a clip out of a jumper to gain better access to pull the splice pin, the knife slipped out of the belt, puncturing the EE on the right leg approximately 4" above the knee, resulting in one suture to close the puncture wound.

Showing all 45EE stated they were cutting a hole in a flapper to get the hook of a chain ratchet in it so they could pull the flapper out of the gob in front of a bottom roller. The knife blade broke, and the knife slipped, cutting the EE on the left palm.
EE was cutting the belt to make a splice, and the knife went through the belt, cutting EE on right leg just above the knee.

The track jack tipped over causing the belt head to sit down on blocking material pinning EE fingers on both hands.

The rail jumped into place smashing the right ring finger between the rail and miner. Employee was treated and released from GVMC. EE returned to work on next scheduled shift, but a follow up visit with surgeon on 9/15/2015 revealed tendons were cut and required surgery to correct.

EE was removing metal band-straps from a pallet of 50-Ton Heintzman Jacks. As he cut the last metal band-strap, one of the jacks sprung out and landed on his right foot. EE sustained deep contusion.
Employee was using a punch lock banding tool to splice waterline and while tightening band the band broke causing his momentum to shift forward and the banding tool struck him in the mouth. Employee received 4 sutures on inside of lower lip and lost 2 teeth.
