Employee was trying to loosen a fitting on a brake accumulator. When fitting broke loose, employee's hand contacted the work bench, and the finger was lacerated by the wrench on the inside of EE's finger. Employee was wearing gloves. Laceration required 3 stitches.

EE was cutting rubber skirting with a box cutter style knife. The knife slipped and cut through the welding gloves they were wearing. EE received 5 sutures to close a laceration on the back of their left hand above the thumb.

EE was tightening a nut on the a frame of a haul truck with a high torque gun, when the torque gun came of the nut and struck EE's finger between the gun and the truck frame.

Operator of haul truck noticed the GPS/Dispatch unit was not fastened securely. The employee decided to cut the zip tie to prevent damage to the unit, then call maintenance. The employee used a pocket knife to cut the zip tie, the knife went through the zip tie then into the thumb side of the palm of employee's left hand.

Employee was using a ground stake pounder to pound a stake in the ground. Employee got right ring finger in between the stake and the pounder, resulting in a laceration on the top of finger. Employee was taken to a local hospital and received 9 stitches.

Employee was attempting to cut a plastic zip tie with a pocket knife. The knife slipped causing a laceration to the employees left pinky and ring fingers. Sutures were required to close the wound.

Miner was removing a portion of the front hydraulic control valve with a chain fall from a CAT 993K wheel loader when the valve slid off the mount striking the miner on the left tibia breaking the bone.

Employee was using a metal shaving tool, transferring from machine lathe to metal bin. In the process of transferring the metal shaving got caught causing the tool and shaving to contact the hand. Employee received deep cut to pinky finger.
