Employee was cleaning snow off conveyor belt walkway, after task was completed, employee removed EE's lock from Substation and proceeded to walk away from substation and slipped on an icy area twisted/fractured EE's ankle, weather conditions snowed the previous day, so the 9th was a mixture of snow and mud.

EMPLOYEE WAS WELDING IN A COAL CAR AND EE SLIPPED AND FELL.
Mechanic lost balance from the cab floor deck and fell forward on the right-side and landed on the track grouser. The mechanic hit EE's head on the side of the engine panel.
Employee was performing preshift inspection on weld truck. Employee had placed left foot on the rung and was lifting right foot to the top when the ladder rung broke, causing the employee to fall. Both knees struck the front bumper before employee landed on employee's feet. Employee was evaluated before transporting to Urgent Care
Employee was holding onto a combination wrench while another employee used an impact wrench to tighten bolts on an 18"" valve. While tightening the bolts, the combination wrench unexpectedly moved causing EE's right index finger to become wedge between the wrench and valve flange, resulting in lacerations.
Employee was cutting some gasket material with a utility knife towards self. The knife slipped on the material and lacerated EE's left palm below the pinky. Employee found the supervisor who called MRT's and met MRT's at the safety building who cleaned and bandaged the wound.

After using a chop saw to cut six pieces of 6'x 1-½"" square tubing, an employee was manually unloading the tubing onto a metal table. While carrying and dropping the tubing over a small MIG welder onto a welding table, the burred edge of the pieces cut into the base of the employee's left thumb.

Employee was operating a motor grader and the right door started to rattle. Stopped to re-close the door. While attempting to re-close the door, fell resulting in ankle fractures.
An employee was found unconscious and unresponsive in the cab of a grader. The employee was transported to the hospital, and was pronounced deceased due to a heart attack.
While retracting a manual pull/push outrigger for a service truck, an employee's finger was pinched between the outrigger housing and the edge of a base mounted vice on the bed of the truck. Employee was wearing hand protection. Contact resulted in a laceration requiring stitches.
Employee was placing a 75-pound dragline tooth on the edge of the parts boat. The right-hand middle fingertip was smashed and cut.
Employee was using jaw pliers to pull off the jacket from a high voltage cable. pliers slipped and struck employee on nose causing a laceration requiring 4 stitches.
On 08/29/2022, employee was pressure washing model 844K rubber tire dozer. As the employee stepped off the dozer push arm, stepped on a two-inch hose and twisted their right ankle. Released to return to work the same day of the injury. MRI scan was rendered and a complete diagnosis by the doctor, determined that the employee could not return to work on 09/29/2022.
An employee wanted to record a crack on the rail and how much it would worsen as the locomotive traveled over it. The employee crouched down in a baseball catcher position to record. The locomotive approached the location where the employee was position at a speed of less than 10 mph when the corner of the first step landing hit the employee in the knee.
While cleaning light fixture inside and out, using a hand towel, ran right middle finger across metal flange cutting the tip of it. Employee received four stitches.

Cutting the side wall of a Dragline Bucket, on final cut, the plate spun and hit EE's leg
The employee stated EE was making a homemade part to repair a sink that was spraying water on the wall. EE was cutting a safety cone with a utility knife to make this part. While cutting EE reached EE's left hand inside the cone and stabbed EE's left thumb. The cut required four stitches.
Employee was driving back to shop on main haul road. Approaching Dixon ramp 2 they got too close to the edge of ditch and got pulled in causing truck to overturn on its side. The employee was wearing a seat belt. EE received a small laceration to the side of head requiring 3 staples.
Employee received a laceration and fracture to thumb when a come-a-long struck thumb.
Two employees were removing left side bull gear carrier pins from an 8750 dragline using a ""canister"" tool. While using this tool and attempting to line up all thread, the pipe slipped, putting the employee's ring finger in a pinch point.
Employee completed a pre-shift inspection on a CAT 785C water truck. Employee was wearing insulated gloves. Operator picked up the wheel chock block and was installing it in the holder located on the front bumper of the truck. The left middle finger tip was caught between the chock block and the metal chock block holder. Finger tip bone was fractured.

The Yazzie pit is an inactive part of the mine, last coal mined in this area was approximately 2010. There are currently no miners with exposure in this area. A supervisor passing in the area, noticed smoke coming from an outcropping and determined that it wasn't extinguishable in the 30 minute time frame.
Employee was riffling a 14 pound sample of coal. As they were dumping the sample they felt a sharp pain in the middle of the lower back. Co Worker helped them sit down and MRT was called. Employee was taken to hospital given pain medication and restricted duty.
Employee was placing a 8""X8""X24"" timber underneath a dragline bucket jewelry spreader bar and pinched left index finger. Employee was wearing leather glove at the time of the event. EE left index finger tip was fractured.

On August 10, 2017, employee hit left elbow jarring left shoulder. After further doctor evaluations, it was determined the employee will need surgery. 1st day missed August 25, 2017.
Employee was sitting in supervisor's pickup and had some type of medical episode. Employee was unresponsive, CPR was initiated immediately. Employee was pronounced deceased by Aircare paramedics.
Employee was aligning a 6"" clay valve and strained their back.
Employee was positioning a floor jack under a loader, while pushing jack forward the jack handle was unlocked, accidentally. Which caused the employee to fall forward, placing left hand on loader and the long jack handle smashed/broke left index finger tip.

Employee replaced cutting tip on Plasma Cutter machine, triggered the nozzle into left palm of hand.

Employee was tightening wheel lug nuts, when the impact trigger contacted the wheel lock ring. It activated the air impact gun wrench causing the employee's left hand to be smashed. Left hand was position on the socket extension, hand was smashed against wheel hub.
Employee was Air Arcing on the bucket and felt debris get into right eye
Security identified open flames (spontaneous combustion) at North Dixon Hiwall on 8b coal seam at station 141+50. It was determine that the fire will not be extinguish within 30 minutes however, we call out an operator and supervisor to respond. The open flame or fire was extinguish at 6:20 AM. No work activity in Dixon Pit, mining and stripping were shut down for the weekend.
Miner was removing a portion of the front hydraulic control valve with a chain fall from a CAT 993K wheel loader when the valve slid off the mount striking the miner on the left tibia breaking the bone.
Employee was using a metal shaving tool, transferring from machine lathe to metal bin. In the process of transferring the metal shaving got caught causing the tool and shaving to contact the hand. Employee received deep cut to pinky finger.
Employee was removing metal scrap from the metal scrap bin. Upon lifting a heavy iron, started to slip from hand and smash left ring finger against the metal bin edge.

Employee was carrying a belt return roller on his shoulder. When walking by the Eriez magnetic separator, the magnetic attracted the metal roller. Employee's middle and ring fingers were smashed.
Employee was descending a ladder, slip and fell, upon landing right ankle fractured.
Employee was raising and tamping #1 track. Employee was resetting track jack and a rock got lodged in the release. His left hand was on the top of the jack body. When the rock was removed, the jack released, dropped, pinching his left hand and cutting his finger.
Employees were attaching the additional rigging when one of the eyebolts that were used to attach the rigging to the motor base came loose. This caused the sling with all the rigging to be thrown into the employee standing on the catwalk. The employee was hit on the hard hat and above the right eye.
Employee was installing a hydraulic hose, in the process of repositioning his body, his foot slipped , employee grabbed the angle iron, causing laceration to right his palm.
Individual was just arriving for duty and parked her vehicle in the visitors parking lot. Individual grabbed her bag and put on her PPE's. The individual closed the vehicle door and began walking toward the walkway near the jersey barriers when she slipped on black ice and fell down on her side. The individual injured her ankle. During muddy conditions employees were driving through a left curve and proceeded straight at approximately 5 MPH. The vehicle slid sideways down into a small concrete wall. After they hit the wall, driver felt pain in her left elbow and pain in her back. Passenger had little neck pain. Vehicle damage was dent/scratch marks to driver side bed and gas tank cover broken off.
Nightshift supervisor was conducting preshift inspection and found the highwall fire in the pit. No work/mining activities on weekends and location of fire determined the flame would not be addressed within 30 minutes.

Employee stopped his 785 Cat End Dump at a stop sign. Another end dump that was following collided with the park End Dump. The employee in the stopped End Dump sustained neck strain. Employee was given prescription drug for pain.

While using electric appliances employee rested her left hand on microwave while pushing down the toaster level. Employee felt a shock go through her left hand and arm.
Employee was installing a new seat, in a CAT Enddump cab. In the process of pulling the seat he lost his grip and fell back into the cab firewall with his left shoulder.
Employee was lowering a dozer belly pan, which stopped an inch from the ground. Employee reached under and grabbed the pan to jarred it loose. The pan loosen and fell an inch. Employee pinky and ring fingers were smashed, to which it required stitches to close the wounds.
Yazzie pit has been inactive for a couple of years. Some coal seams are expose to ambient temperature and environment. With recent monsoon rain a spontaneous combustion occurred on 8 seam coal.
Employee was position under the dozer, removing outer equalizer pin with drift punch and a maul. On the backswing the maul hit the coil diverting the hammer backswing into upper lip.
Welder using air chipper felt discomfort in right eye. Reported discomfort and eye was irritated.
Employee was removing a loader bucket tooth. In the process of using a sledge hammer to strike the tooth. Metal debris hit the employee in right leg thigh.
Employee (electrician) was troubleshooting an electrical fault in the sample system. In the process of opening the circuit breaker it arced and flashed causing burns to cheek and ear.

Employee forgot his key to his tool box lock. He did not have access to his tools to work for the day. He decided to use a 1/2"" chuck electric drill and 3/8"" drill bit to drill out the lock. While drilling, the bit bound up and twisted his hands. The twisting fractured the 4th metacarpal on his right hand.
EMPLOYEE WAS AT MINE REPROGRAMMING RADIOS. HE PUT HIS WIRE CUTTERS IN FRONT JACK POCKET AFTER CUTTING TIE TO REMOVE RADIO. Employee adjusted the loader hydraulic limits and while stepping between hand rails felt pain in his left shoulder.
Employee was driving on the haul road and discovered a rock in the roadway. He tried to manually pick up the rock and felt a sharp pain in his bicep.

In the process of shutting the Door (Dozer), smashed right hand finger between door latch and striker.
Employee does not know exactly how he received his injury. Dust or Dirt got into his eye and was treated by doctor.
Employee was walking with traffic on a public road that accesses the mine site. A vehicle was traveling on the public road that accesses the mine site, a collision occurred between the vehicle and employee.
785 Caterpillar end dump truck drove into the back end of 16M Caterpillar motor grader. While the motor grader was blading the haul road the end dump had straddled the windrow. End dump driver indicated that he did not see the motor grader. Our investigation concluded the driver was distracted (distraction is unknown).

After washing off the oil accumulation on his equipment, at the wash bay. Employee was walking towards washer wand storage bracket. He slipped on ice and landed on his left knee.
Employee was walking towards changeroom in the change room building. She felt a pain in her left knee after she missteped on ramp. Employee worked until March 9, 2011, had surgery on the torn menious in the left knee.
Maintenance Mechanic was conducting a field visual inspection of a coal hauler. He knelt down to look underneath the equipment felted a minor pin-like penetration of pain(Right Knee). He did not feel the need to report until February 10, 2011 after his knee and leg swelled from infection.

The wrong tool for the task, a long handled sledge hammer (10lb) was used to set railroad spikes for the spike driving crew following. The primary hazard was not identified during the SLAM Process. The employee also got complacent and lost focus of hand placement. The result, the employee lost the index finger nail and recieved 3 sutures.
Employee was attempting to climb on PL63115 Stacker stepped on a chunk of coal, lost her footing and twisted her right knee. First day of missed work was on January 31, 2011, due to surgery.
After completing his dozer pre-op, the operator was adjusting the armrest to his comfort. The armrest was not sliding with ease. Operator grabbed the armrest with both hands and rocked the armrest, while applying a downward pressure. The armrest suddenly gave and the left middle finger was smashed between the armrest and metal dash. The cut required stitches to close the wound.
Employee was jarred in the cab when loader was dumping coal into the coal hauler that she was operating.
Employee was welding & grinding inside the tub section of the dragline. When he exited the tub to the outside, wind blew foreign bodies into his left eye.
Employee was applying upward pressure to 4 inch all thread with a 5 foot pry bar. The bar slipped off the all thread causing the employee to fall from the second step of the ladder he was working from. The employee struck his left knee. The employee has been precribed meds & restrictions. (These were for a similar injury at their own Shop and do not apply to this report)
WORKER WAS STACKING CRIBBING FOR THE DRAGLINE AND A PIECE OF CRIBBING ROLLED OVER AND SMASHED HIS RIGHT MIDDLE FINGER BETWEEN TWO PIECES OF CRIBBING.
Employee was removing a steering cylinder from a Letourneau loader. Both pins were removed and had a come-a-long installed to assist in lowering the component down. The lifting strap was installed somewhere in the middle of the str cylinder barrel. When the steering was rocked loose from position the cylinder swung and crushed the left hand of the employee.
Tripped over washdown hose while washing a conveyor tunnel. The walkway has a metal grade (expanded metal). When employee fell his finger got caught in the expanded metal and over extended his finger. The over extending cause the skin to rip apart.
Walking out on shop apron step on frozen mud. His knee shifted and while lifting his other leg, placed upper body weight on left knee. Felt a pain in the knee joint and visited a doctor. The incident happened on February 5, 2010 however had surgery done on March 2, 2010 and started the miss day from schedule.

Employee was removing a pulley on an alternator when the impact wrench trigger was accidently pushed and spun the pulley. The employee crushed his finger between the pulley fins and a bolt. His finger was lacerated and required sutures.
Metal shavings embeded into welder's left eye while replacing an equalizer pin on a dragline bucket.
During a pre-shift inspection, the operator was stepping onto a dragline bucket when he heard a pop and sprained his left knee.
An operator hit a rut in the road that caused the loader to bounce. The operator hit his head against the glass that required 2 staples to close the laceration.
Employee over extended his knee while dismounting a dragline shoe step. The distance between the step and ground was approximately 3 feet to the ground.
Employees were hoisting a dozer blade cylinder when the top section came apart and swung and hit the other mechanic in the forehead that was prying the bottom end loose. Employee received 2 staples to close laceration.
Employee lacerated finger when it was pinched between a reclaimer belt through roller and frame. Employee received 5 sutures.
Employee fractured left ankle when he twisted it on uneven ground.
Employee had shoulder pain after grabbing a hand rail when he lost his balance while standing on a loader battery box. After a MRI was done, it was determined that damage was done around the shoulder rotary cuff. Employee was put on restricted work on April 8, 2009.
Employee first day missed was 2/09/2009. At approximately 1700 hrs a mechanic was installing a cover for the rear axle box on a loader. The axle cover weighed approximately 25 lbs. The mechanic was sitting on the floor lifting the cover into place when he felt a pull in his lower left side.

An employee fell 4 feet from the deck of a lowboy while dismounting a front end loader vertical ladder.
Employee was unloading a pipe-threading machine with another employee when the machine slipped. The falling machine jerked the shoulder of employee. Employee missed first scheduled shift on 9/5/2008.
A hook on a come-along that an employee was using broke from its hooked postion and hit the employee on the lip and nose.
Employee was lifting a grease drum up to drill deck. There were two employees on the drill assiting when the drum was let down on the deck before the employee had moved his hand from beneath the drum. The drum pinched the employee's left hand injuring the index finger.
While returning to the pit, the operator missed the turn and hit a safety berm on the haul road while operating a coal truck. The operator stated she fell asleep after the event.
Employee was putting tools away in a Jobox, when the lid came down and smashed his right hand. A fracture was not revealed until March 3, 2008 when a MRI was done.
He had been installing pins on a dragline. Our crew had stopped for a short break. Someone from another contracting let loose a comealong with out notification wrench triggered a chain to slip and that chain went over ee's thumb crushing it.
Employee was cutting jack plates from the bottom of the revolving frame when the plate came loose and it bounced on the tub and hit the employee on the right leg just above the knee cutting the leg.
Employee was grinding on the mast with double eye protection with another employee grinding to his right. The wind blew some trash into right eye.
Heavy equipment operator was riding in a buddy seat all shift while task training another operator. Operator reported his back was injured. Employee did not notify his employer until 1/16/2008 at the start of shift.

Employee was in the process of checking the gap between a dozer push arm and a trioun with a pry bar. The weight of the push arm pulled the pry bar, which then jerked the employee's arm.
An employee was placing a 4X4 tubular metal to push back into the metal steel rack. While placing the tube down, his finger was pinched and smashed between a corner of a sharp edge and the tubular metal.
Maintenance D employee was struck on the right hand above the knuckle by a hand crank pump pulley handle as he was raising the pump.
Employee was attempting to replace a guard on a swing motor for the 8750 dragline. He potentially received an electrical flash/shock.
While removing windows for cleaning, the frame of the window came apart. As the individual reached for the falling window, the window came in contact with the floor, his hand struck the bare glass and was cut requiring 4 stitches.
While shutting down the electrical switch gear in sub station 5 an arc occurred and ignited a small amount of coal dust. A yellow flame was seen by a witness.
Employee tripped in meeting room, twisting her left knee.
Ee was tramming a Genie manlift when he drove into a small ditch. This caused a jarring motion, up and down, to his back. Ee felt pain and was determined he strained his lower back.

Employee completed washing a dozer when he stepped in a pothole in the washbay. As a result, strained his right knee.
Foreign particle embedded in left eye. Employee's eye became irritated after reboarding his truck after the loader completed loading large rocks. The particle was found the next day and removed by medical provider. Due to continued irritation the employee did not report next scheduled shift.
Employee was struck on his hard hat by a piece of falling coal that spilled out of the crusher above him while he was cleaning on second deck of plant.
Employee was operating a D10 dozer to lower the face height and push down large rocks from the cap rock for the loader. The operator was developing a key cut against the highwall side when he struck a large rock that caused the blade to skim over and as the tracks advanced, the dozer tipped over on its side.
EE was welding & grinding on top of DL Tub. A high wind came up causing the fire blanket, being used as welding shields, to whip violently. Threads from the fire blanket got into EE's eyes. Washed them out at site but because of nature of material elected to take him into clinic to be sure all removed. Everything okay.
Employee was cutting a 3"" x 8' piece of metal. It was lopsided & fell & hit the instep of his foot. Badly bruised but no fractures.
EE was grinding... flipped up face shield w/out leaning forward & all caught on rim fell into eyes. Washed out rt eye & thought had gotten all out. When EE woke next morning lt eye matted. Went to eye clinic where rust ring was found & removed.
Employee sustained injury to left thumb (open wound & fracture) by pinching thumb between the mast and adjusting tool at completion of drill repair.
Trip & fall. When entering a trailer used for temporary security post she tripped on a wheel well cover inside the trailer. Fell & injured right knee. Released to sedentary work 4/10/06. Recheck 4/12/06. Released to sedentary work.
Employee was working on rail car cable. He was trying to reach the J-box to dial in new car settings when he missed a step while climbing over hitch area. He fell backwards onto the air brake actuator cylinder assembly resulting in fractured ribs.
While lining up ties, employee hit the top of his foot with a six foot long bar.
On January 16, 06, a welder reported an eye injury incurred on Jan 13th while arc welding and grinding wear plate on the 8750 dragline. At the end of dayshift Friday his left eye was irritated. On Jan 14th using a mirror and flashlight he noticed a piece of metal in his left eye and went to the San Juan Regional Hospital ER where the metal was removed.

EE removing drill bit from Lamina..locked up to change bit..Lt hand on drill bit when lock released falling on hand smashing Lt index finger between drill & table. EE working alone..co-worker had gone into base to spot-face hole. Usually work 2 men to change bits & extensions.
Ee was using a pry bar to put rims together on the Kress truck tires. EE pinched his left thumb between the rim and the pry bar causing a laceration of 1/2"" resulting in 3 stitches.
EE was lifting 12x12x2 metal plate to sit on top of a hydraulic track when he felt some discomfort in his lower back. He went to Dr on 8/3/05. Was allowed back to work with restrictions. Put him to working in toolroom with NO LIFTING. Worked remainder of 8/3 & 8/4 until 10:00am. He decided to go home because back hurting.
Preliminary report - At approximately 12:30 p.m., Ee was backing down an incline with a mix truck loaded with ANFO. The truck took off, hit a berm, and flipped on its side. EE had scrapes on her elbows at time of incident but a couple of days later, said she had dizziness.
EE was assigned to load grease barrels for off-site shipment on a flatbed truck. As required, she obtained a disposable respirator as part of PPE application. After an hour of usage, she noticed irriation to the sealed area of the face. A rash appeared with some numbness and some swelling. She was transported for medcial evaluation for the allergic reaction.
The employee was ripping Overburden material in Dixon Ramp 4. During a ripping pass, he turned to his right to observe his ripping sequence & he felt pain in his left rib area. The hard OB material he was ripping contributed to his injury.
EE was in process of assisting in the removal of transport safety block on dragline bucket. He was assigned to cut the safety braces on the left side of the bucket. As he positioned himself, he was verbally told to hold up. As he began to climb down he placed his hand on the spreader bar and the hoist wire rope moved and pinched his right index finger and middle finger.
EE complained of headache due to fumes.
(2 employees were injured in this accident.) The 2 employees were buddy-welding a new 8750 drag line base. Both torches had been left partially on (1: Oxy 2: Oxy/Prop). When one EE struck an arc to weld there was a flash from the escaped gas that burned both employees. This EE had burns on his face and ear.

EE was in process of unhooking the trail cable off the tub hooks of the 8050 dragline with SE-3137 cable tractor when his right index finger got caught in between the cable tractor ROPS and the cable tie off hooks on the left front side of the machine.(rt index finger @ 2nd knuckle)
EE was in process of using a come-along to move a piece of metal plate associated with the #1 crusher screen. As he reached under the screen to unhook the come-along the metal plate slid and pinched two of his left fingers, which resulted in 3 stitches on his left index finger. The extra slack in the chain stopped the metal from falling fully onto this fingers.
The EE was working on a pulling fixture for removing a pin when he felt a pain in his left eye. The EE was chipping a weld with a chipping hammer. Subsequently, a metal particle became lodged in his left eye. He was wearing required PPE.
As EE was repositioning scaffolding during his welding assignment, he moved a fire blanket which cuased dust particles to be become airborne which got into his left eye.
The EE was in the process of moving a radiator shroud of a D-11 dozer out of the way on the shop floor to make room for additional equipment to drain anti-freeze. As the EE moved a shroud which was slightly propped up against the dozer track it slipped out of his hand and fell approximately 6-7 inches pinching his left middle finger.

The employee was assigned to clean up after a major repair job on a Kress Coal hauler in the main shop. The employee was throwing scrap metal pins in a waste bin. As he threw a 2nd pin in the bin, a large seal bounced back and struck the employee in the mouth. His upper lip was lacerated and received 8 stitches. In addition, he chipped three upper teeth.
The employee was egressing from his work station (down the coal plant control tower stairway) to perform hopper cleaning duties. As he was coming down the stairway he twisted his knee and heard a pop in his left knee as he stepped down from the last step. His left knee was termed a strain at the medical facility and was returned back to work without restrictions.
A NON INJURY OCCURED WITH AN APPROX. 400'SECTION OF HIGHWALL, SLOUGHED OFF INTO & UP AGAINST THE COAL HAULER. THE EMPLOYEE WAS NOT INJURED. COAL MINING ACTIVITIES IN THE NORTH PIT. PUMP 3 WAS SUSPENDED & EQUIPMENT & PERSONEL WERE REMOVED FROM THE PIT. EXTERNAL DAMAGE OCCURED TO THE COAL HAULER. MSHA AZTEC OFFICE WAS NOTIFIED THE NEXT MORNING.
Employee was laying out surface delays when he stepped over a surface trunkline with his right foot and buckled his knee. He lost his balance momentarily but didn't fall. His right knee twisted when the soft drill cutting gave way while transferring his body weight from his left foot to his right foot.
Employee was removing a damaged 3"" x 3"", 10 foot angle iron from the top level of PLO05170 Stacker at the Coal Plant. Employee's right finger was caught against another angle iron causing laceration to his index finger.

AFTER PRE-SHIFT INSPECTION AT THE DRAGLINE, EE WAS SITTING IN THE DRAGLINE OPERATORS CAB & TOLD CO-WORKER THAT HE HAD A HEADACHE. WHILE CO-WORKER WAS OPERATING THE DRAGLINE EE CAME BACK IN FR OM THE OILERS ROOM & TOLD CO-WORKER THAT SOMETHING ELSE WAS WRONG WITH HIM. REALIZING THE EE WAS EXPERIENCING PERSONAL MEDICAL CONDITIONS, CO-WORKER SHUT DOWN DRAGLINE & CALLED FOR MRT'S.
Reported late on 3/31/03: while loading a hoist pedestal pin into a maint. truck the EE slipped in a muddy area he stepped into and strained his left knee. The EE initially reported to his shop supervisor on 3/27/03 and (allegedly) reported to the 8750 outage supervisor on 3/31/03. The EE was given first aid treatment and returned back to full duty the same day.

WHILE TIGHTENING A BOLT NEAR THE HYDRAULIC LINE ON TRD5321 DOZER, EE HIT THE COOLANT HOSE CAUSING THE ANTI-FREEZE FROM THE HOSE TO DRIP INTO HIS LEFT EYE. HE SUSTAINED CORNEAL ABRASION IN HIS EYE. HE RECEIVED FURTHER MEDICAL CARE AND RETURNED TO FULL DUTY ON NOVEMBER 5, 2002.
DURING SWING SHIFT, EE WAS HEATING UP A BOWL OF SOUP FOR LUNCH. IN THE PROCESS OF REMOVING THE BOWL FROM THE MICROWAVE, EE SPILLED THE SOUP ON HER CHEST AREA SUSTAINING FIRST AND SECOND DEGRE E BURNS. SHE RETURNED TO WORK FULL DUTY WITH NO RESTRICTIONS ON OCT. 29, 2002.

SHE SUSTAINED MINO R LACERATIONS TO HER FOREARM; CONTUSIONS AND INFLAMMATION TO HER KNEE AND A CERVICAL THORACIC STRAIN. SHE RECEIVED MEDICAL TREATMENT AND WAS RELEASED TO FULL DUTY. SCHEDULED FOR CHIROPRACTOR.
WHILE EE WAS SERVICING GRD191 GRADER AT DIXON PIT, HE STEPPED OFF THE TANDEM AFTER CHECKING THE ENGINE & TRANSMISSION OIL. HIS FOOT SLIPPED OFF THE STEP & FELL TO THE GROUND. AS HE FELL, HE GR ABBED THE HANDHOLD BUT FELL ONTO THE WIGGINS FUEL NOZZLE PROTECTIVE COVER & BROKE 2 RIBS ON HIS RIGHT SIDE. HE RECEIVED FURTHER MEDICAL ATTN @ MESA OCCUPATIONAL & SPORTS MEDICINE.
WHILE MOVING THE DOZER JACK STAND, FROM UNDER UNIT, EE'S LEFT THUMB WAS CAUGHT IN BETWEEN THE DUAL TILT CYLINDER AND THE JACK STAND RESULTING IN A LACERATION OF HIS UPPER LEFT THUMB.
THE AIR HOSE TO THE CHIPPING FUN WAS PUNCTURED CAUSING A LOT OF COAL DUST TO BLOW AROUND. ON JULY 13, 01 SHE REPORTED BOTTOM RIGHT EYE WAS PUFFY AND PAINFUL WHEN SHE BLINKS WHILE WORKING AT HER COMPUTER IN HER OFFICE.

WHILE ASSISTING TO REMOVE THE REAR DRIVELINE ON TRW335 RIMPULL COAL HAULER, EE PLACED HIS RIGHT HAND ON THE U-BOLT THAT SURROUNDS THE DRIVELINE. THE DRIVELINE DROPPED APPROXIMATELY 8-9 INCHES ONTO THE U-BOLT PINNING EE'S FIFTH RIGHT FINGER AGAINST THE THE U-BOLT. A CONTRACTOR EE WAS ASSISTING EE WHEN THIS OCCURRED. WHILE REMOVING LEAD STEEL FROM EDD 226 DRILL EE S FOREARM WAS CAUGHT BETWEEN THE CAROUSEL AND THE HOT END OF THE STEEL WHEN HE WAS PUTTING ON A 1/2"" CHAIN AROUND THE LEAD STEEL.
WHILE EE WAS STEMMING AND TYING IN DIXON COAL SHOT ON MON MAY 8TH, 2000 DUST PARTICLES BECAME IMBEDDED IN HIS LEFT EYE. THIS ACCIDENT WAS A RESULT OF EXTREME WINDY CONDITIONS. HE DID NOT REPOR T THIS ACCIDENT UNTIL WED MAY 10, 2000. HE WAS TREATED BY MESA OCCUPATIONAL SPORTS MEDICINE WITH AN ANTIBIOTIC OINTMENT AND RELEASED TO RETURN TO WORK.
