Employee was trying to free blockage of rock/material in a tail pulley using a pry bar. The pry bar slipped and the employee fractured EE's right ring finger. Equipment was de-energized and locked out when employee performed this task.

Showing all 2Employee states was working with a come along when employee felt a pop in shoulder. The employee did not report the accident to supervisor. When the supervisor became aware of the allegation the employee was taken to the clinic for a medical evaluation and released with no prescription medication, told to rest shoulder and return for a follow up exam in a few days.
