Employee was cutting pump discharge line with a knife. As employee was cutting, the knife cut through the line causing a laceration to left thumb. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Employee was attempting to break bolts loose on a miner head by pulling a ratchet toward self. While doing this EE felt a pop in EE's left shoulder. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

While cutting a rock dust hose employees knife slipped cutting into ee's hand. This did not involve any compliance issues, job proficiency, attitude, or machinery.

While lifting a pump from sump to clean, the pump became unhooked from the chain hoist and fell on top of employee's left foot. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency, attitude, or machinery.

Left index finger got pinched between sledge hammer handle and gear box casing while trying to remove a coupler with hammer. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency, attitude or machinery.

While re-railing a supply car employee felt a burning sensation in abdomen while using a track jack. Employee continued to work until the doctor took EE off work starting 7/24/2017. Employee, while pulling down on a 24" pipe wrench to loosen nut, felt a pop in left upper arm. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency, attitude, or machinery.

While installing bolt in fish plate, employee had hand under rail while another employee was holding rail up with a slate bar. The rail slipped off of the slate bar sliding down onto employees fingers. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency, attitude, or machinery.

Employee claims to felt a "pop" in right shoulder while tightening a hose while working under heavy equipment. Cause of this injury was stretching too far to pull on wrench. Compliance with rules/regs., PPE, or mining equipment/systems not a factor. Miner proficiency may be a factor.
