EE was working in an unsafe position while changing out a reducer on a Scoop. While come-a-longing the reducer up, the reducer slid over and hit the side of EE's right foot. ***-- This report was submitted earlier under the wrong Mine ID, 1519945. This is the correct Mine ID and report. Thanks.

Employee was cutting tape off of chuck with a belt knife, and cut their hand in the process.

Injured was changing tire on lo trac and as EE let the jack down, EE had their hand on top of tire. As jack dropped, the finder of lo trac caught EE's hand between finder and tire causing a laceration and bruising. 4 stitches were necessary to close laceration.

EE stated that EE was cutting conduit with EE's knife when the knife slipped causing a laceration to the left index finger. The laceration resulted in 7 stitches. EE did not lose any time and was not duty was not restricted.

EE STATED THAT A ROCK WAS STUCK BETWEEN THE BLADE OF THE SCOOP AND BUCKET. EE WAS USING A 3 FEET LONG PRY BAR TO REMOVE THE ROCK. WHEN THE ROCK CAME LOOSE THE WEIGHT OF THE BLADE DROPPED ONTO THE BAR, CAUSING THE BAR TO PIN EE'S FINGER BETWEEN THE BOTTOM OF THE SCOOP BUCKET AND THE BAR. EE RETURNED TO WORK ON LIGHT DUTY.

EMPLOYEE STATED THAT THEY WERE REMOVING A BEARING RACE FROM A MINER GEAR BOX WITH A 4 LB HAMMER WHEN A PIECE OF METAL STRUCK THEM IN THE LEFT LEG. EMPLOYEE WAS TREATED AT THE HOSPITAL AND RECIEVED ONE STITCH.

EE WAS ATTEMPTING TO DRIVE THE CAP BACK ON THE GEAR BOX OF THE MINER WITH A CRIB BLOCK WHEN THE CAP FELL OFF MASHING EE'S LEFT THUMB. EE WAS TREATED AT THE HOSPITAL FOR A BROKEN THUMB. EE RECIEVED STITCHES AND RETURNED ON LIGHT DUTY.

While attempting to install a track idler on the continuous miner EE was guiding the idler into place when the come along slipped and fell onto EE's left hand. EE was transported to Harlan ARH, where EE was x-rayed and 8 stitches were put in EE's pinky and ring finger.
