EE stated that EE was cutting conduit with EE's knife when the knife slipped causing a laceration to the left index finger. The laceration resulted in 7 stitches. EE did not lose any time and was not duty was not restricted.

EE WAS GETTING A SHUTTLE CAR TIRE OUT OF A RAM CAR WHEN EE TWISTED A KNEE AND THE TIRE STRUCK RIGHT KNEE/ANKLE AND FOOT. EE CONTINUED TO WORK UNTIL 2-16-22 WHEN SURGERY WAS SCHEDULED FOR 4-11-22.

EE WAS ROOF BOLTING WHEN A PIECE OF DRAW ROCK FELL AND STRUCK RIGHT FOOT/ANKLE. EE SAW A DOCTOR AND WAS ALLOWED TO CONTINUE WORKING UNTIL 11-18-21 WHEN IT WAS DETERMINED THERE WAS A FRACTURE OF THE END OF THE FIBULA. EE BEGAN TO LOSE TIME ON THAT DATE AND IS CURRENTLY OFF WORK.

EE stated that EE twisted a knee as EE was turning to walk to the continuous miner on 10-5-21. EE continued to work and consulted a doctor. An MRI was done and EE had surgery on 4-7-22. This accident became a lost time accident on 4-7-22.

EE STATED THAT EE HAD FELT EE'S KNEE SHIFT TO THE LEFT WHILE PICKING UP BELT STRUCTURE IN MUD. EE STATED THAT EE FELT PAIN AND CONTINUED TO WORK. ON 9-20-21 EE SAW A DOCTOR THAT PUT EE ON LIGHT DUTY PENDING SCHEDULING AND RESULTS OF AN MRI.

EE was tramming roof bolter when EE ran over broken mine bottom & caused head to hit canopy of bolter. EE was placed on light duty until EE visits orthopedic dr. Further investigation revealed roof bolter was being pushed by scoop after getting stuck on slick, broken bottom. EE stated EE was in operators deck attempting to tram bolter & head struck canopy of bolter during process.

EMPLOYEE STATED THAT THEY WERE REMOVING A BEARING RACE FROM A MINER GEAR BOX WITH A 4 LB HAMMER WHEN A PIECE OF METAL STRUCK THEM IN THE LEFT LEG. EMPLOYEE WAS TREATED AT THE HOSPITAL AND RECIEVED ONE STITCH.

EE WAS ATTEMPTING TO DRIVE THE CAP BACK ON THE GEAR BOX OF THE MINER WITH A CRIB BLOCK WHEN THE CAP FELL OFF MASHING EE'S LEFT THUMB. EE WAS TREATED AT THE HOSPITAL FOR A BROKEN THUMB. EE RECIEVED STITCHES AND RETURNED ON LIGHT DUTY.

EE was assisting in working on the take reel of the roof bolter. EE had been positioned on the top of the roof bolter and was getting down when EE twisted EE's knee. EE continued to work until 12-16-20 when EE had surgery for a torn cartilage.

EE stated was building a ventilation stopping when EE's back popped while EE was lifting or picking up a concrete block. EE continued to work and came outside as the pain increased in EE's back. EE was taken to the Dr. and lost the first day of work on 8-11-2020 pending an MRI which is scheduled for 8-25-2020.

EE was loading supplies onto the roof bolter machine when EE felt pain in EE's back. EE continued to work until 8-27-2020 when EE was taken off work pending an MRI.

EE STATED THAT EE HAD DUMPED A LOAD OF POND SILT AND WAS TRAMMING BACK DOWN THE HILL FROM THE 14 CURVE WHEN EE APPLIED THE BRAKES BUT HAD NONE. EE STRUCK THE BERM TO SLOW/STOP THE TRUCK. EE WAS TRANSPORTED TO HARLAN ARH AND WAS TREATED AND RELEASED. THE DR. DETERMINED THAT AN MRI IS NEEDED AND TOOK EE OFF WORK UNTIL RESULTS ARE READ.

EMPLOYEE STATED THAT ON 6-22-20 EE WAS ASSISTING IN HANGING MINER CABLE FROM THE ROOF WHEN THEY FELT PAIN IN LOWER BACK. EE CONTINUED TO WORK AND SAW A DR. ON 6-25-2020 WHO DETERMINED THAT AN MRI IS NEEDED AND TOOK EE OFF WORK PENDING THE RESULTS.
