The handle hit the mast cutting/pinching the employee's right index finger. The employee received three stitches at the emergency room, and the doctor cleared the employee for full duty.
2 Employees were in the process of replacing a Lip Protector on a CAT 992 Loader. During the Process one employee grabbed a hammer and struck the tip of a tooth, to help engage the tooth. The tooth chipped and a piece of metal struck the employee on the left cheek imbedding metal in cheek.
Employee was in the process of cutting drip line on leach pad with a razor knife, while cutting the plastic line the knife slipped, making contact with the left forearm, causing a laceration to the forearm,requiring stiches to close.
