While positioning a drill shock sub to attach it to the drill head of a ZT44 drill, employee sustained a crushing injury to three of their left hand fingers. This was caused when the drill steel wrench closed on the employees fingers.

Employee was pressure washing the underside of the cone crusher. EE was utilizing a 5' ladder, the ladder shifted as EE was descending the ladder causing EE to lose EE's balance falling backwards off the ladder landing on EE's back. EE's left leg became entangled between the rungs of the ladder. Employee had a back contusion.

Employee was using 2 wrenches to tighten a bolt on a strip vessel. One of the wrenches slipped of the bolt and struck the employee on the upper lip. This caused a 1/4" laceration.

Worker went to doctor with reaggravation from previous injury on 2/22/2023. Worker sought medical treatment on 5/16/2023 when off and contacted Manager with a note for restricted duties. Worker had previously been cleared for full duties from the 2/22/2023 injury.

The employee swung a hammer, missed the target, and the momentum carried the swing beyond the target to the drill mast. The handle hit the mast cutting/pinching the employee's right index finger. The employee received three stitches at the emergency room, and the doctor cleared the employee for full duty.

Haul truck experienced neck pain when getting loaded with ore. The injury was Tuesday but was first aid care. Employee went to the hospital on Wednesday and was given 2 days off work but ended up being Restricted work for two days of Thursday and Monday.

A haul truck operator was dumping a load at the crusher, and when the operator was lowering the truck bed, it caused a rocking motion which jarred the employee's back. Initially, first aid only. EE put on restrictions on 03/01/2023 making this reportable.

In the process of moving a length of 4 inch HDPE pipe, the employee lifted the pipe over EE's head and walked forward, employee lowered the pipe to the ground. An hour later the employee felt a pop and pain in the stomach and groin area.

2 Employees were in the process of replacing a Lip Protector on a CAT 992 Loader. During the Process one employee grabbed a hammer and struck the tip of a tooth, to help engage the tooth. The tooth chipped and a piece of metal struck the employee on the left cheek imbedding metal in cheek.

Attempting to clear a blockage of AN from the bed auger utilizing a pressure washer. The first maintenance worker asked the driver of the truck to test the auger, the second maintenance worker had opened the auger inspection door and reached inside as the driver started the auger. Resulting in Amputation to the Index & Middle Fingers from the second knuckle on the left hand.

An operator of a CAT 777D Haul Truck was parked to be loaded by a CAT 992G Loader. The first bucket of material contained a large rock. When the bucket was loaded into the haul truck, the large rock caused the truck to shake. This caused the haul truck operator to experience neck pain.

Worker states EE was gathering wear iron on the lower waste dump to stack onto wooden pallets. Worker states as EE attempted to move the fourth piece of wear iron it slipped out of EE's hands. The worker states EE attempted to catch the piece of iron with EE's left knee and felt a slight pinch in EE's lower back and pain in EE's knee.
