EE using hammer to loosen a bull hose from cam fitting. Other EE assisting and pulling on bull hose. Bull hose released unexpectedly while the EE was swinging hammer, and their thumb was pinched between the two pound hammer and the cam fitting. At ER, thumb nail was removed and one suture placed in nail bed. Small fracture at tip of thumb. EE released to restricted duty.

Employees removing pins with a pipe and hammer. Technician 1 was striking the pipe while Technician 2 held the pipe in place with their hand. After striking the pipe the pin was pushed out, causing the pipe to go into the bore, at the same time, the weight of the loader adjusted causing technician 2 to have their thumb pinched between the pipe and bore.

Ee was using cutters to strip back insulation on a wiring harness. The cutters slipped, striking the ee on the right inner forearm. Seen at NNRH where the laceration was closed with staples and sutures and EE was released to modified duty. EE follows-up on 9/30.

EE stripping motor leads with stripping tool. Cable jacket difficult to remove, so they took off a glove to use their thumbnail to peel jacket off. Gripped loose jacket with left hand, while holding stripping tool with right hand. Pulled jacket, when it broke free, left hand struck stripping tool, causing laceration. Seen at ER, laceration sutured, released to restricted duty.

EE was assisting another EE lifting a cathode. The prybar dropped and they went to catch it and felt pain in shoulder. Seen at clinic, released with restrictions, and referred to ortho. Diagnosed with possible rotator cuff tear and referred for MRI, continued on restrictions.

Ee was working to break sub loose on drill steel. The wrench they were using turned in the opposite direction the ee expected it to, and it struck the ee on the right ankle. Ee seen at the ER, diagnosed with a fracture, and referred to ortho. Released to restricted duty.

Ee was chipping slag off door with a chipping hammer, and a piece of slag flew up under EE's safety glasses and got in EE's eye. First aid was provided and ee seen at the ER, released to full duty, and referred to an eye doctor. Eye doctor also released to full duty. Diagnosis corneal abrasion, prescription given.

Ee was using a pry bar to help guide a cylinder into place while standing on the third rung of a 6 foot step ladder. As another operator moved the cylinder into place, it caused the pry bar to "kick back", causing the ee to lose balance and fall from the ladder, landing on left hip. Ee transported by ground ambulance to hospital, orthopedic surgery done on pelvis.

EE was using a safety knife to cut a piece of hose when the blade broke on the knife, causing the hose to slip and the knife to cut their hand. Seen at health clinic, diagnosed with a laceration that was closed with stitches, and released with restrictions. Working modified duty.

EE was using rap wrench to break bonds on a bundle of spiling (pipe). Pushed wrench on last bond, bond broke, causing the pipe to roll down, pinch ee left middle finger. Seen at ER, xray negative for fracture, sutures required to close the wound.

Ee using box cutter to cut trailing cable shielding. Knife tip broke and went through cut resistant gloves, cutting the ee's thumb. Seen at ER, sutures required, ee released to full duty.

EE reports feeling difficulty moving neck (pain/stiffness) after using a wrench to tighten a bolt. Pain and stiffness continued. No specific incident, site cannot recreate incident. As of 10/2 workers' compensation has not accepted claim, however due to EE's report of pain, NGM is reporting to MSHA.

A RAM belt technician was performing a finger splice on a conveyor belt, the OLFA blade broke in the rubber of the finger splice. The employee obtained a laceration on right thumb, the broken blade punctured through employee's level 6 cut glove. The employee received 4 stitches, tetanus shot, and given a full duty work release.

The injured worker used a 24" pipe wrench to loosen a joint. The joint was near eye level, and the employee placed the pipe wrench on the joint at a vertical angle and began to pull downward. In the process of pulling downwards the wrench slipped and hit the employee in the lip. There was a small laceration on the lip that only required first aid; however, the tooth was fractured.

EE was using comealong to pull a belt up when the hook came off and hit their lip. The employee did not seek treatment for the injury, but NGM determined that the EE's real tooth was chipped, prompting this report.

Ee was torquing nuts on a valve. Ee was adjusting the open-end wrench and when ee began to torque, the wrench moved and pinched ee's pinky finger between the torque and the open-end wrench. Seen at clinic, diagnosed with distal fingertip fracture, released to full duty, referred to hand specialist.

Ee was working with another person putting a drive line back on a carrier bearing. The weight of the drive line shifted, and the ee's hand was caught between the drive line and the come-along. Seen at clinic, laceration closed with stitches. Seen again on 10/9, released to full duty. No restrictions, no lost time.

EE reports cleaning mud off concrete with a snow shovel, shovel got stuck in crack and the handle struck them in the right side. Diagnosis chest wall contusion. X-ray confirmed no fractures. On follow-up on 8/28, EE released with restrictions, making this reportable.

EE changing wear iron on loader. In the process of hammering the wear iron off the loader, a piece of shrapnel dislodged and struck the EE in the left leg. Seen at ER, referred to surgeon. Shrapnel surgically removed, EE put off work pending post-op appointment.

The crew was prepping for a top head change and decided to try to pry the spindle tube free when the spindle tube became unstuck it dropped IP stuck EE's hand in a pinch point to grab the fallen spindle tube causing a cut to EE's ring finger on EE's right hand.

Lube tech was changing a filter on a CAT 777D haul truck and caught their hand between the ratchet handle and a hydraulic hose causing an avulsion to the first knuckle joint of their right hand requiring 9 stitches to close.

A crew were conducting sonic sampling. The injured miner was holding a bag to collect soil sample while second minor was striking the core barrel to release sample. The injured miner's hand slipped into the path of a 2lb sledge hammer and was struck. The injury was diagnosed as a laceration to right, middle finger. Follow up appointment, 1/9/2023, reveled non-displaced fracture.

Seen at Humboldt General Hospital, diagnosed with right index finger distal phalanx fracture and laceration. Sutures, splinted, released to modified duty.

DURING CLEANUP FOR THE DAY WHILE CUTTING RUBBER ROD FOR FLOORING, CONTRACTOR CUT SELF WITH UTILITY KNIFE ON THE WRIST. A BANDAGE WAS APPLIED BY THE EMPLOYEE AND EE WAS NOT CONCERNED, FINISHED CLEANING FOR THE DAY AND LEFT SITE. ON THE WAY HOME DECIDED TO HAVE IT CHECKED OUT BY URGENT CARE, RECEIVED 8 STICHES.

Ee was using a post pounder to set ground rod. The post pounder slipped off the top of the rod, smashing finger between the rod tip and post pounder handle. Seen at ER, diagnosed with a fracture to the finger tip, and referred to hand specialist. Ee released to modified duty, no use of the injured finger, pending orthopedic consult. Site can accommodate.

EE was helping extract a sheared off lead anode from a EW cell. EE was pulling anode from below floor level with a hook when they felt their left shoulder pop and then had pain and numbness in fingertips. Seen at medical and diagnosed with a shoulder strain. Released to restricted duty that site can accommodate.

Ee was using a chisel to remove a broken bolt from the base of a pump when a piece of bolt broke off and went into EE's left eye. Seen onsite by emergency response and requested treatment on 12/30. Diagnosed with a corneal abrasion and referred to optometrist. Released to full duty.

EE was reinstalling a clamp on hose and used a pry bar to close the clamp. EE pulled on the pry bar with both hands and felt a sharp pain in EE's lower right side abdomen. Seen at Golden Valley, diagnosed with a hernia. EE is being scheduled for surgical repair of the hernia.

Ee was scaling off a hanger plate. A loose piece of shotcrete fell and hit the ee in the left eye and on the nose. Ee was seen at the ER, diagnosed with a corneal abrasion, blunt eye trauma, and iridoplegia. Ee was referred to an eye doctor and released to restricted duty on ee's next scheduled shift.

Ee was attempting to move a table by pulling on a vise. The vise was not mounted and was pulled off the table, landing on the ee's left foot. Seen at a med. center, xrays negative for fracture, ee put off work and referred for consultation with a foot surgeon.

Employee was conducting maintenance on a dozer. While utilizing a prybar to turn the engine, the bar slipped and caught left pinky between the engine and the prybar. The employee removed glove and discovered a small abrasion, employee was examined at the hospital and it was identified that the pinky suffered a fracture on the distal end.

Ee was using a screwdriver to clear a blockage in a shotcrete accelerator hose. The blockage was scraped away, and backed up material sprayed the ee in the face. EE flushed eyes, EMTs flushed EE's eyes. EE sought treatment the following day and put off work due to corneal abrasion and med requirements.

Walking by bay 1 while someone was hammering on a bucket. Something flew off and hit employee in the face. Thought it had hit a tooth, then noticed the bleeding. After EMT cleaned up, noticed it was through and through. Sent into town for X-rays. X-rays showed metal still in lip it was removed.

Two mechanics were setting sump pump. They thought it was on the grating, but when the come along was released, the pump twisted and caught ee's hand between the pump and grating guard. Transferred to SLC due to unavailability of orthopedist in Elko. Seen at ER, partial amputation of fingertip, ee released to light duty.
