A maintenance crew was working on separator rotor cage. While lifting it with steel chokers, one of the steel chokers failed. Consequently, the other choker broke causing the cage to drop approximately 3 feet. As the cage fell, a maintenance repair got EE's fingers between the cage and the fan shroud where EE's hand was sitting, causing severe lacerations to 3 fingers

Employee stated they were pushing on the handle of a come along when they felt a sharp pain in their right shoulder. Employee was placed on restricted duty and PT on 12/11/23.

Employee lacerated right index finger while cutting a zip tie. Employee received stitches to close wound

Employee struck right hand with 10lb sledge hammer while clearing a chute. Employee did not seek medical attention until 11/8/22. Xrays showed fracture in the hand.

While attempting to cut a 36" conveyor belt with a razor knife, a maintenance repairman lacerated the back of left thumb. This resulted in stitches to close wound.
