Showing all 4The employee was climbing down a scaffold ladder. Employee was tied off to an overhead SRL while descending a ladder. Their personal srl got caught on the scaffold gate. While descending the ladder, they noticed the caught psrl hook. As they looked up at it, the hook freed itself and retracted as designed. In that retracting process, the hook hit our employee in the mouth.
While changing bits on a boar miner the bit wrench slipped smashing their left thumb between it and the tooling plate requiring four sutures.
Employees were pulling a feed side conveyor chain into place with a chain fall hoist, the hook shaft broke and pulled out of the knuckle ring assembly. When this occurred, the employee was struck in the elbow causing a one-half-inch laceration which required two (2) sutures.
