Showing all 40Miner was scaling with a scaling bar and a rock fell onto the scaling bar and the weight of the rock resulted in downward momentum of the bar and the employee pinched their right hand between the scaling bar and the mine floor resulting in a fractured pinky finger and metacarpal.
Miner was attempting to get a socket seated onto a corroded nut on a vic coupling by hitting the socket with a pipe wrench. EE missed the socket and struck EE's left thumb with the pipe wrench.
EE was using a knife to cut some plastic off of an oil containment tote when the knife struck something and kicked out of the plastic. The downward force of the knife struck EE's leg causing a laceration.
An employee was removing a large nut from the torque arm on the gear box on conveyor, when pulling on the wrench the employees left middle finger popped. The employee did not seek a medical evaluation at that time. On 12/10/2020 the employees finger was swollen and was not improving so EE was taken to a medical facility for evaluation at which time EE was put on restricted duty.
EE was removing the last bolt from the sheave liner and had one hand pulling on the ratchet and the other holding a bar so the sheave wouldn't turn. EE went for more throw on the ratchet and started to turn the bolt to loosen and right pinky finger got caught by the bar.
EE was using a pry bar to try to adjust liner plates on a bin feeder in the crushing plant. The tip of the bar slipped down to where a moving pan feeder contacted it and caused the bar to lever forward crushing EE's left hand between the bar and the bin feeder.
Employee was cutting a couple of inches off slicker pants, as employee cut with the blade coming towards body, knife slipped and cut left thigh.
The employee sprained wrist while attempting to loosen a wing nut on a 4" bull hose with a hammer. The employee was initially treated and released to full duty, as of 5/19/2016 the injured required a cast and restricted duty to assist with healing of the tendon.
While tightening a bolt at head level, the wrench slipped striking the employee in the tooth resulting in a broken front tooth.
The employee received a minor laceration on his right thumb while using tin snips to cut thin sheet metal.
The employee received two minor lacerations to the back of his left hand while attempting to cut off and plastic handle on a water jug.
EE was helping the miner hang wire in the stope. The dwyidags they were hanging the wire from were rusty and when they tried to tighten the nut on the bolt, rust flaked off and became airborne. The particles swirl around in the ventilation and got around the employees safety glasses.
On 07/01/2014 E was pulling on a wrench with his right hand to loosen a nut. The wrench slipped and fell on his left hand pinching his left pointer finger against a bolt. On the night of 07/08/2014 the finger started swelling and turning red. E reported the incident on the morning of 07/09/2014 and was taken to medical care giver and received antibiotics to treat infection.
Employee injured his left shoulder(A/C ligament) while removing a bolt from a victaulic coupling. Bolt was rusted and employee was using a ratchet with socket to remove the nut when he felt a pain in his shoulder.
In the process of digging a sump ditch, employee bent over to pick up a pick axe. When he was standing up, he twisted at the waist and felt a pain.
Employee was tightening a bolt when the ratchet slipped of the end of the bolt head and hit him in the upper lip. Employee required stitches to close up the wound.
Worker was using a chain binder to secure plate steel to forklift. As the chain was tightened it rolled over pinching worker's thigh against the steel plate.
Workers were lowering a work staging. They disconnected a chain fall and the staging tilted to one side causing material to slide across the deck striking the worker on the upper arm.
Employee attempted to open a stiff air valve using a pry bar. The employees' hand was placed in a pinch point and his thumb was struck by the pry bar when it slipped. The employee was taken to the clinic for treatment. A x-ray of the hand revealed a hairline fracture at the tip of the thumb. The employee was released back to work with no restrictions.
A mechanic was hammering slag off of a loader bucket, when a piece of metal flaked off of the hammer and embedded itself in his hand, leaving an 1/8" hole. The procedure to try and remove the metal resulted in an incision that required sutures.
While mucking the miner felt something in his left eye. A piece of rock was embedded in his eye.
A mechanic was trying to remove a nut off of a bolt in a vise. The wrench broke free and the mechanic struck his elbow on a metal workbench lacerating it.
Miner was barring a rock out of 5970 loading cell and felt a sharp pain in his right shoulder.
Our employee was using a set of bolt cutters to cut some chain, he was using his knees to help aid in the cutter when his right knee popped out. EE has been working full duty in his regular job since the incident. He had surgery on 12-22-08, thus making it a reportable injury.
An electrician was trying to open a pocket knife that was jammed. The knife blade broke loose and the electrician cut his thumb. Five stitches were required to close the wound.
Employee was removing black plastic off of a pallet of lime. he was holding the plastic with his right hand and cutting with his left. He missed a piece of plastic and had to recut, the knife was switched to his right hand and cut towards himself cutting left wrist
While unloading grizzly from the cage employee got his glove caught on the grizzly section. This resulted in his smashing the tip of his middle finger on his left hand in between the grizzly section and the side wall of the cage. He was taken to the doctor and received 7 stitches.
Two employees were loading a round. While one was tamping the powder he felt pain in upper right arm. He finished shift and worked next shift and noticed bruising on his upper right arm. When he came to the office he was taken to the doctor and an appointment was scheduled for 7/23/07 for re-assessment. On 7/25 an MRI was taken; on 7/30 the results made this a reportable accident.
EE was stripping 410 electrical cable with a knife. The knife slipped off the cable and punctured his left hand. Puncuture required one stitch.
Employee was walking past two employees who were pounding a pin into a rockbreaker with a double jack. Upon striking the pin, a small piece of steel broke off the edge of the double jack and struck his arm and became embedded. It had to be removed and sutured.
EE was lifting a battery out of a Jumbo drill w/a lifting strap. As he got about chest high, the right clamp slipped off and came up & struck him in the right eye, causing trauma to the eye w/ damaged blood vessels.
Employee was pounding on the side of a jaw crusher with a sledge hammer and smashed the tip of her finger.
EE was barring down loose rock from the back. a slab came down & hit jackleg drill & fell onto the bar he was using and had dropped. the end of the bar was laying across the boot of his right foot. the force of the slab hitting the other end of the bar forced the end over the boot down on to his foot.
EMPLOYEE WAS INSIDE BALL MILL HELPING CHANGE LINERS. ANOTHER EE STRUCK ONE OF THE BALL MILL LINERS W/A DOUBLEJACK. WHEN HE DID A SMALL PIECE OF STEEL CAME OFF THE LINER & STRUCK THE INJURE EE IN THE SIDE OF THE NECK. THE WOUND REQUIRED SUTURES.
EMPLOYEE WAS CUTTING A HEAD BOARD FOR SAND WALL WITH HAND SAW AND A PIECE OF SAWDUST GOT IN HIS RIGHT EYE. IT BECAME IMBEDDED AND RESULTED IN 1 DAY OF RESTRICTED DUTY.
WHILE HE WAS BREAKING ROCKS ON THE POCKET, A ROCK ROLLED DOWN AND HIT HIS FOOT.
EE WAS PULLING ON LARGE WRENCH W/CHEATER PIPE & PULLED MUSCLE IN RIGHT SHOULDER. ORIGINALLY WAS ONLY FIRST AID. HE HAS HAD PROBLEMS WITH IT AND RECENTLY ENDED UP HAVING TO HAVE SURGERY FOR THE TORN MUSCLE, THEREFORE MAKING THIS REPORTABLE.
STRIIKING STEEL WITH A HAMMER WHILE DRIVING B.O STEEL OUT OF DRILL CHUCK. SLIVER OF STEEL CHIPPED AND ENTERED LEFT HAND BETWEEN THUMB AND 1ST FINGER.
