While a miner and EE's partner were building a sand wall, the center cap came down coming in contact with the miner's leg. Resulting in a hairline fracture to the tibia.

Employee was hand mucking the south end plate in the 2 shaft when a rock rolled pinching left hand resulting in a tuft fracture and a laceration to left middle finger.

Miner was scaling with a scaling bar and a rock fell onto the scaling bar and the weight of the rock resulted in downward momentum of the bar and the employee pinched their right hand between the scaling bar and the mine floor resulting in a fractured pinky finger and metacarpal.

Miner was sealing a new airlock door and the door came into contact with left ring finger causing a tuft fracture to the fingertip.

While performing the daily safety checks on the hoist, the hoist operator pushed the emergency stop button to test the weight brake and the hoist tripped on an E-stop fault. Electricians assisted the operator in resetting the hoist. Hoist was back online at 5:53 pm.

Miner was attempting to get a socket seated onto a corroded nut on a vic coupling by hitting the socket with a pipe wrench. EE missed the socket and struck EE's left thumb with the pipe wrench.

At 4:15am the hoistmen was attempting to clutch in when the weight brake set and EE could get it to release. Maintenance was called and found that a safety was tripped causing the hoist to dump air pressure setting the weight brake. maintenance reset the safety bringing the hoist back into operation.

Employee was operating an open cab haul truck and while turning a strand of wire from the wire mesh ground support on the rib caused a puncture wound to the back of the employee's left shoulder requiring two sutures.

Silver Shaft hoist operator heard noise coming from the drum while the hoist was in operation. The hoist operator called maintenance to come inspect the hoist and they found that a saddle and both nuts on a Crosby clamp had come loose. The clamp that came loose was one of 4 extra clamps that were added to hold the tail of the rope on the inside of the drum.

Employee was trimming the front wallplate with a battery powered saw and while making a cut, the saw slipped causing a laceration to employee's thigh. Employee received 8 sutures.

The hoist tripped due to the gap on the right brake stand-2 upper caliper was detected on the hoist control system to be outside of tolerance at 2mm. The gap was checked and adjusted to 0mm at which time the hoist was reset and back operational. The Silver Shaft Hoist (primary) and 2 Shaft hoist (secondary) were operational throughout.

Lost incoming power to the mine site for a short period of time due to a weather event which caused the hoists to trip. The hoists and compressors needed to be reset and started back up. Hoists were back operational by 7:04 PM.

Employee was transporting a tote of diesel fuel and got a flat tire on the 1-yard forklift they was using to transport the fuel. Employee was pulling into a cutout to get it out of the roadway and as they were pulling in they hit a bump and reached their arm out toward the rib and strained left shoulder. Employee ended up having to get surgery for a rotator cuff injury.

Miner was mucking into a muck bay with a 2-Yard LHD. They had a full bucket and when attempting to dump it they got on uneven ground and as they raised the bucket the loader tipped over onto the right side. After it tipped, oil and fuel began to leak onto hot components of the exhaust and possibly the turbo which caused the fluids to ignite, resulting in the loader catching fire.

When the hoistmen tried to reset the Service Hoist it would not reset and gave a YA1 fault. The hoistmen called electricians and while diagnosing they found a bad fuse. Electricians replaced fuse and got the hoist operational.

EE was installing a 18" split set with a Jackleg. As the EE was pulling the jackleg out of the bolt, the jackleg pivoted and pinched EE's hand between the jackleg and the wingnut.

At 2:20 the hoistmen tried to start 2 shaft hoist but it would not start. Electricians were called to diagnose problem and found the pinion break pressure was at 70 psi which needed to be 1500 psi to start the hoist. It was determine that the oil filter was partially plugged causing the issues.

EE was manually moving a barrel of oil onto the containment when it started to tip. EE tried to stop the barrel from tipping and got hand caught between the barrel and a steel I-beam.

Driller and helper were adjusting the height of the control panel while the driller was getting ready to insert the pin the panel was released and smashed EE's thumb causing a crushed fingernail that required 4 stitches.

EE was re-installing the guard onto the tail pulley of BC-5 when the guard slipped from their hands and pinched 2 fingers on right hand.

EE smashed right thumb between lock bar and cage door on the top deck of the service hoist. EE was attempting to open the cage doors while it was stopped at the 5900 level when the incident occurred.

It was determined that the YA1 converter breaker was defective and needed to be replaced. After consulting with ABB's drive specialists and changing out the breaker, the service hoist was tested multiple times without any erroneous YA1 faults and was deemed back in good operation.

At 10:30 pm there was a report that 2-shaft top station was hazier than normal but gas readings with a Ventis pro 5 gas detector were normal. A supervisor bump tested an MX6 gas monitor & accompanied employees back to 2-shaft portal to investigate. When the 2nd air door was opened, there was smoke so they retreated & initiated the mine emergency response plan.

EE pushed the 4' split set bolt into the drilled hole in the back by hand. As EE was positioning self to drive the bolt the rest of the way in with the jackleg, the bolt fell out of the hole and struck EE on the right forearm.

The compressor plant went down causing the mine to lose compressed air. The brakes on the hoist would not release because it takes compressed air to release them.

EE was removing an electrical jumper cable from the rib and felt a "pop" in EE's stomach. EE was diagnosed with a small hernia. EE continued mining at full capacity. EE elected to have a procedure to fix the small hernia on 7/3/2023. EE returned to work after procedure on EE's next scheduled shift of 7/8/2023.

A breaker for the battery backup for the #2 hoist tripped during a severe weather event during the night. This caused the batteries to wear down because they could not charge due to the tripped breaker. Once the battery level got low enough, it tripped another breaker to the Grinnell System. This caused the hoist to shut down.

EE was struck in the face by a rock that was dislodged from the face. The miner was installing a bolt into the fingers of the bolter when the bolt hit the face, a rock slide down the bolt and struck the employee.

EE was operating a haul truck on the mainline when they felt something go into their eye. EE was taken to the eye clinic and checked out. EE was given a full release at that time. On 11/21/2022 they contacted the eye clinic with discomfort in the injured eye. EE was given an antibiotic to treat the eye making it a reportable injury.
