Injury was not reported until 9/6/13. EE alleges that he felt a pull in his stomach while adjusting the under belt on the twin screen. He has been diagnosed with a hernia and released back to full duty but will require surgery at a later date. The accident date is a just a guess by the EE.

CRUSHER CHUTE PLUGGED UP. EE USED A 5 FT BAR TO TUG A PIECE OF CONCRETE FROM BEHIND. IT WAS PINCHED BETWEEN THE CHUTE AND GOOSENECK BOLT. THE PIECE CAME LOOSE AND THE CHUTE CAME DOWN ON THE BAR AND SPUN IT AROUND & HIT EE ON THE SIDE OF THE HEAD.

EE WAS UNPLUGGING THE JAW. A ROCK FELL AND SPLIT. EE GOT CHIP IN HIS EYE.

EE WAS USING A MAUL AND FENCE POST TO BREAK UP ASPHALT IN THE JAW. THE HANDLE OF THE MAUL PINCHED EE'S THUMB BETWEEN THE MAUL HANDLE AND FENCE POST.

CHIPPING DIRT OFF A STEEL PLATE WITH A 3' PRYBAR. BAR SLIPPED, CAUSING CRUSHING INJURY TO LT. SMALL FINGER.

WHILE CUTITNG 36 INCH BELTING FOR RETURN ROLLERS, KNIFE SLIPPED AND E.E. WT WRIST.
