While prying the bearing loose, EE's right pinky finger was caught between the prybar shaft and the metal edge of the "top-head" on the right. EE's pinky was smashed causing a laceration. EE was taking to the emergency room and received stitches.

Showing all 34Employee was attempting to put a bullet screen on a loading hose. Employee struck the bullet screen with a metal hammer, causing one of the ears to break off and hit them in the mouth and chip a tooth.
Employee was tightening the lid on a tank car with a wrench and the wrench slipped out of EE's hand and EE felt a pain in their lower back. The employee initially tried to treat with first aid but pain worsened and EE wanted to seek medical attention. Employee was seen at local ER and diagnosed with a back strain and taken out of work.
Employee was using a tool bar to close the bottom door on a hopper car. The bar slipped out of the cog causing EE to mash their finger between the bar and the concrete.
Employee was installing bar grating on a steel frame. EE was using a two foot pry bar to slide grating into place. While pushing the pry bar with left hand, EE felt a sharp pain in left bicep. The force from the pushing of the pry bar, caused an internal tear to the lower left bicep of EE's arm.
Employee was performing maintenance of a spray dryer. while holding a wedge to remove a bearing with a hammer, EE missed the wedge and struck EE's hand.
The employee was lifting a pipe with a crow bar and the bar slipped causing the pipe to fall on the employee's left ring finger causing a cut that required stitches.
While clearing a crude lump from the feed chute to processing equipment the shovel the employee was using came into contact with the processing equipment and pinched the employee's left hand between the shovel handle and the nearby equipment guard.

The employee was attempting to remove a reducer bushing from a 75HP motor junction box when the chain wrench slipped causing it to strike EE's left pinky finger.

USING VICE SECURED BY ROPE TO PULL PUMP FROM WELL. ROPE BROKE AND RECOILED BACK HITTING EE IN FACE UNDER RIGHT EYE CAUSING INTERNAL BLEEDING TO EYE AND PAIN.
