While prying the bearing loose, EE's right pinky finger was caught between the prybar shaft and the metal edge of the "top-head" on the right. EE's pinky was smashed causing a laceration. EE was taking to the emergency room and received stitches.

Employee was attempting to put a bullet screen on a loading hose. Employee struck the bullet screen with a metal hammer, causing one of the ears to break off and hit them in the mouth and chip a tooth.

Employee was tightening the lid on a tank car with a wrench and the wrench slipped out of EE's hand and EE felt a pain in their lower back. The employee initially tried to treat with first aid but pain worsened and EE wanted to seek medical attention. Employee was seen at local ER and diagnosed with a back strain and taken out of work.

Employee was using a tool bar to close the bottom door on a hopper car. The bar slipped out of the cog causing EE to mash their finger between the bar and the concrete.

Employee was installing bar grating on a steel frame. EE was using a two foot pry bar to slide grating into place. While pushing the pry bar with left hand, EE felt a sharp pain in left bicep. The force from the pushing of the pry bar, caused an internal tear to the lower left bicep of EE's arm.

Employee was performing maintenance of a spray dryer. while holding a wedge to remove a bearing with a hammer, EE missed the wedge and struck EE's hand.

The employee was lifting a pipe with a crow bar and the bar slipped causing the pipe to fall on the employee's left ring finger causing a cut that required stitches.

While clearing a crude lump from the feed chute to processing equipment the shovel the employee was using came into contact with the processing equipment and pinched the employee's left hand between the shovel handle and the nearby equipment guard.

2 employees were removing a bearing race. Once the race was hot, the employee stepped back and removed welding hood as the second employee started striking the race with a hammer. A piece of metal broke off and struck the first employee in the bottom lip causing a laceration that needed stitches.

The employee was attempting to remove a reducer bushing from a 75HP motor junction box when the chain wrench slipped causing it to strike EE's left pinky finger.

The employee was cutting duct tape off of a vacuum hose with a knife and while doing so the knife slipped and the employee cut left hand on index finger resulting in 2 stitches.

INJURED EMPLOYEE WAS WORKING WITH ANOTHER EMPLOYEE WHO WAS USING A PRYBAR ON A PACKING GLAND. PRY BAR SLIPPED AND STRUCK EMPLOYEE'S SAFETY GLASSES CAUSING AN ABRASION TO RIGHT EYE AND CONTUSION TO FACE.

APPLYING PRESSUE TO A 24" PIPEWRENCH WHEN HAND SLIPPED CAUSING WRENCH TO STRIKE EE IN CHIN REQUIRING STITCHES.

EMPLOYEE CUTTING STRING ON BAG WITH KNIFE WITH LEFT HAND AND CUTTING WITH RIGHT. KNIFE SLIPPED AND STRUCK LEFT THUMB RESULTING IN LACERATION REQUIRING STITCHES.

HELPING OTHER MECHANICS CHANGING MOTOR ON PUMP. WHILE CUTTING TAPE OFF PUMP MOTOR HIS KNIFE SLIPPED AND CUT THRU GLOVES AND CUT LEFT 5TH FINGER REQUIRING SUTURES.

EMPLOYEE WAS ATTEMPTING TO UNSTOP HOT CLAY FROM PUG MILL WHEN HE WAS BURNED UNDER HIS ARM FROM ELBOW TO HAND AND FELL ON KNEES RESULTING IN BRUISES.

Cutting a tie strap off a hose on a John Deere Tractor. While cutting the strap, the pocketknife slipped cutting the foreman on the left thumb/hand area.

FITTING NEW HOSE ON OIL PUMP BUCKER USING KNIFE WHEN HE CUT HIS RIGHT THUMB.

STANDING ON LADDER, PULLING ON 1/2" DRIVE LONGHAND RACHET. RACHEL POSITIONED UPSIDE DOWN. SOCKET SLIPPED OFF NUT, STRIKING EMP ON RIGHT SIDE OF MOUTH REQUIRING STITCHES.

Employee was trying to get spur gear cover off #4 Filter. He was using chisel and sludge hammer. As sludge hammer struck chisel, the chisel shot out and hit employee in chin resulting in laceration requiring stitches.

EMP WAS TIGHTENING BOLTS ON AN ELLIOTT MILL ROTARY ASSEMBLY, WHEN WRENCH SLIPPED CATCHING HIS THIRD FINGER ON RIGHT HAND IN A PINCH POINT, RESULTING IN A FRACTURE TO TIP OF FINGER.

EMPLOYEE WAS CUTTING A 1 1/4 INCH RED RUBBER HOSE WITH A KNIFE WHEN KNIFE CLOSED ON RIGHT HAND MIDDLE FINGER CUTTING IT REQUIRING 6 STITCHES.

WHILE CLEANING OUT OVERFLOW PIPE WITH IRON ROD, EMPLOYEE STRUCK HIMSELF ON THE LEFT KNEE, CAUSING JOINT EFFUSION.

EMPLOYEE WAS BREAKING A PIECE OF BANDING WITH PLIERS. BANDING BROKE AND HIT LEFT HAND CUTTING AND REQUIRING STITCHES.

Employee was removing sleeve from pump and shaft with a punch and a hammer when piece of metal flaked off the punch and entered his left arm.

Employee was repairing a water leak at 2 & 3 Tracks, using a ratchet to tighten flange bolt. The ratchet slipped off the bolt and struck him in the upper lip, requiring stitches.

Employee was using a pipe to open a waste line quick open valve and got his finger caught between pipe and concrete resulting in a fracture to his distal phalynx on right third finger.

WHILE DRIVING A PIN IN BOLT HOLE TO LINE UP, THE EMPLOYEE WAS STRUCK BY A PIECE OF FLAKE FROM HAMMER. THE PIN WAS STRUCK BY A COWORKER - THE EMPLOYEE WAS STANDING ABOUT 7' AWAY.

EMPLOYEE WAS CLEANING TAIL PULLEY ROLLER ON INCLINE BELT CONVEYOR. WHILE ATTEMPTING TO TURN ROLLER WITH BAR, BAR SLIPPED CAUSING EMPLOYEE TO FALL FORWARD STRIKING BAR WITH RIGHT ARM RESULTING IN A BREAK TO HIS FOREARM.

USING VICE SECURED BY ROPE TO PULL PUMP FROM WELL. ROPE BROKE AND RECOILED BACK HITTING EE IN FACE UNDER RIGHT EYE CAUSING INTERNAL BLEEDING TO EYE AND PAIN.

USING A HEAVY BAR TO POKE CLAY AT SHREDDER AND FELT PAIN IN MIDDLE OF BACK.

CUTTING RIGHT OF WAY WITH BUSH HOOK AND FELT SEVERE PAIN TO LEFT SHOULDER WHILE SWINGING BUSH HOOK. DID NOT GO TO DR. UNTIL WEEK LATER THINKING PAIN WOULD GO AWAY.

EE WAS REMOVING INSULATION FROM ELECTRICAL WIRE WITH A POCKET KNIFE AND CUT HIS LEFT HAND INDEX FINGER REQUIRING TWO STITCHES.

EE WAS UNLOADING TOTE BINS FROM PICKUP TRUCK. TRYING TO LOOSEN ROPES WHILE USING A KNIFE, ROPE SLIPPED CAUSING KNIFE TO CUT THUMB REQUIRING STITCHES.
