Was holding up skirt board rubber while pulling a belt on when the pry bar came in contact with the tail pulley pinching finger between the pry bar and I beam. Employee was put on light duty or restricted duty.

EE was working on the Telsmith #44 tightening bolts. Wrench slipped off the bolt causing him to fall into the belt guard and motor. He received a scraped shin and buttocks. He did not go to the doctor until 5/20/08.

EE was trying to unplug the crusher using a pry bar and hammer. While he was driving the bar into the material, he hit his index finger on his right hand. His finger was laid open.

Employee was cutting the belt off on M-17 conveyor and slipped. He hit his knee with the belt and slipped and cut his knee. Due to weather conditions, this was a factor in the injury.
