Employee was performing a pre shift walk around inspection on the water truck. He raised the hood to allow for a fluid check. As EE walked around to the other side of the truck he struck his head on the mirror causing a cut at the hairline on his forehead.
EE was cleaning up brush & tree limes from a storm and that had been previously cut to clean fence row. During this process a small twig or branch entered the ear canal and penetrated the ear drum.
Employee was using a ?? coated cable to hoist a pipe out of the back of a pick up truck onto a conveyor. As he was lifting the pipe it started to slip. He let go of the cable but the cable caught on his finger, spraining it.
EE WAS USING A COME-ALONG TO REMOVE A MOTOR FROM A SLURRY PUMP. WHILE PULLING ON THE COME-ALONG THE EYE RING IN THE MOTOR BROKE OFF CAUSING EE'S ARM TO HIT A HAND RAIL.
EE WAS GETTING OFF EQUIPMENT AND MISSED THE BOTTOM STEP.
STRAINED BACK WHILE LIFTING BATTERY ONTO DRAGLINE.
