Employee was pulling on a rock with the scaling bar when they felt a pain in their stomach. They went to lean on the cage and felt a sharp pain in stomach. This became reportable on 03/13/2026.

Employee was scaling inside the preheater on the wall of preheater. EE went to scale some material off the wall, and then felt the right knee pop. EE said felt a discomfort when EE started to walk, and a sharp pain was apparent when EE would take a step. This was felt mostly when EE extended leg walking.

While scaling a rock off of a ledge on the pillar the rock twisted and contacted employee's scaling bar causing the bar to contact EE's right arm and overextended EE's elbow. There was a discrepancy in thought as to the extent of the injury, however, it does appear there may have been a tear.

Employee was struck in the mouth by a 6' scaling bar. The employee was attempting to pull rock with the bar when it slipped and struck the employee in the mouth. The employee suffered a laceration to the lower lip resulting in three (3) stitches. Employee returned to work.

Employee felt small pop in shoulder while swinging hammer. EE finished day and went home. Employee was taken to doctor and released to full duty. Employee received surgery on 03/22/2023 and missed time starting 03/23/2023.

Employee stated was trying to clear material from around a drill steel using a scaling bar at the access door to the chute, when a rock came loose and bounced out the door and struck employee's hand. The employee was administered stitches to close the wound.

Using a crescent wrench to tighten a bolt when the wrench slipped hitting EE's hand on the rail resulting in a fracture of the tip of EE's ring finger

EMPLOYEE STATED WHILE SWEEPING, THE SWEEPING CAUSED THE AIRBORNE MATERIAL TO FALL INTO THE EMPLOYEE'S RIGHT EYE. THE EMPLOYEE WAS TAKEN TO THE LOCAL EMERGENCY ROOM AND FOLLOWED UP WITH THE LOCAL EYE DOCTOR, IT WAS FOUND THAT THE EMPLOYEE HAD A METAL SHAVING IN THE RIGHT EYE. IT WAS REMOVED AND PRESCRIPTION WAS GIVEN. NO TIME LOST.

EMPLOYEE was attempting to cut some insulation plastic with a retractable safety utility knife and the knife slipped with thumb on the retractable part holding the knife open. It cut the front of right thigh. Employee went to local doctor and received stitches.

Employee stated EE was manually scaling with a bar and EE's left wrist became painful and swollen. EE was sent to doctor locally and remained on full duty. EE's wrist became more swollen and was sent to a specialist on 02/24/2017 and the specialist placed EE on light duty. We could not accommodate light duty so EE is currently missing time as of 02/25/2017.

Employee was chipping out grout/lime from beneath a fan on a demolition project and felt debris enter into his left eye. Employee was using a chipping hammer and was wearing all necessary PPE. Employee flushed eye immediately but did not feel that debris was removed so he was taken to an eye doctor. Eye doctor removed debris and provided a prescription for antibiotic drops.

Employee was beating on outside of metal coal hopper with sledge hammer due to coal being frozen. Sore arms, shoulders, thought pain would go away, it didn't. Investigation started 3/5/14. Seen by physician 3/6/14. Placed on restricted duty back to work and is currently working on restricted duty, no time lost.

Employee stated while using a pneumatic chisel for material removal a piece of hydrate went under his glasses and into his eye. He went for treatment where he was given a prescription medication for his eye.

Employee stated he was un-choking a feeder with a hammer and struck a protrusion with his hand as he was swinging. He received 7 sutures and went back to full duty.

Employee stated he was breaking apart a water line with a pair of channel locks and crescent wrench when he felt a muscle tear in his upper left arm. He was given conservative treatment until 04/16/2012 when he had surgery and went on light duty. He continues to work.

Employee stated he was scaling and sat the scaling bar on top of his foot. When they moved the cage the bar contacted the roof and pushed the bar onto his toe. His right middle toe was cut near the joint. He is currently missing time from work.

While scaling, employee stated he had his scaling bar in a crack, when the bar slipped out and he lunged forward. The momentum caused something to pull in his right shoulder. Employee was given several treatments with physical therapy.

While using an allen wrench to tighten a bolt it slipped off and he hit his right forearm on a gear causing a laceration. The laceration required sutures.

While closing a bottom slide on a railcar the bar slipped out of the caption which caused his hand to hit the side of the railcar. He received a minor fracture to his right little finger. *Addendum: X-ray later revealed there was no fracture.

Employee was demoing a casting on a preheater using a com-a-long. material host. when he cut the casting looselt fell and knocked him off. he fell 6 to 8 ft causing injury to his back and leg. we will find out friday 11-5-04 what his job restrictions will be.

Employee stated while using a bar to scale he was struck in the mouth. He did not report to Safety Dept. requesting dental repair until 10/22/2004. I contacted MSHA subdistrict office and they called Ann at MSHA (303-231-5453) who advised to include in this narrative why it was not reported in May, 2004. Per call on 11-2-04, he was struck by the scaling bar- coder 12.

Employee was opening a valve and pulling toward him when the valve wrench slipped off and hit him in the lip. This occurred on Midnights on March 2, 2004 and both I and the supervisor received notification on 04/04/2004 that he received sutures and it was reportable.
