Employee as working on replacing the #1 drive motor for the screen. While torquing the bolt, the motor pivoted, causing EE's left hand to be caught between motor mount and pivot point; resulting in a fracture to EE's left thumb. EE went to clinic and EE's thumb was splinted.

Showing all 6Employee was trying to tighten the tension adjuster for the tail pulley on the feeder. EE was using a 3ft pipe wrench to tighten the nut and applying pressure down when the wrench slipped causing EE to strike chin on the tail pulley guard. Employee received a laceration to their chin that required three stitches.
The employee was performing maintenance on a sand dryer. During maintenance the employee was cutting zip ties off of electrical wires with a collapsible knife. While doing so the employee cut the top of right hand resulting in five stitches.
