Employee was attempting to adjust bolt alignment fingers on bolter using a hammer, while doing so the employees left hand pinky finger got caught between the hammer and the alignment finger causing a laceration. Employee received 7 stitches.

Employee was attempting to seat a clamp and struck right thumb with brass hammer. Employee has a fracture in thumb.

Employee was performing work with a sledgehammer on 1/25/25. Due to increasing pain, medical attention was requested on 1/27/25 and the employee was taken off of work.

Employee had a pipe wrench up to chest while working on a pipe and was trying to use body weight for leverage when employee felt a pop in ribs. On 1/13/25, the employee was taken off of work after further medical evaluation.

Employee's hand made contact when wrench slipped off bolt causing a laceration.

Employee was using a hammer and made contact with their hand resulting in a laceration.

Employee was installing a bushing in a haul cylinder and struck left hand with a hammer. The injury resulted in a non-displaced fracture of the index finger. Employee has been released to "work to comfort level" and is performing light duty and computer work.

The employee had finger pinched between a steel bar and valve handle causing a fracture.

An employee was opening a parts container in the Cashman warehouse. Employee was using a razor knife to break a zip tie on the container. They pulled the knife toward themselves and the container slipped, causing the knife to lacerate their wrist between the coat cuff and cut resistant glove cuff. The laceration required 3 sutures to close.

Employee felt a pop in their shoulder on 3/22/2016 and later noticed a deformity with their arm. It was determined that EE had a rotator cuff and biceps tear but surgery was not needed at that time. Due to continuous loss of strength in that arm, the employee requested surgery which occurred on 5/18/2020.

As an employee was cutting zip-ties off of PVC pipe, the knife slipped, puncturing the employee's left hand. The employee was transported to Nye Regional Medical Center where they received 1 staple. The employee was released to full duty.

Employee was breaking fittings loose on bottom winch line on deck with a 1 1/4" wrench to repair a hydraulic leak when wrench slipped and hit him in the face chipping two front teeth.

An employee was stripping PVC wire insulation away from the inner conductor with a razor knife. The employee cut through their leather glove and lacerated their left palm and wrist. The employee was transported to the RMGC Clinic where they received 11 sutures. The employee was released to restricted duty.

While changing out an equalizer bar on a D10R Track Dozer, an employee had his hand in a pinch point between the equalizer bar and Dozer frame while he was using a wrench. The employee's left pinky and ring finger were pinched between the frame and equalizer bar resulting in a crushing/amputation injury to these two fingers.

An employee was opening a container of drill steel coupler anti-seize with a knife and the knife slipped causing a laceration to the employee's left inner thigh. The employee was transported to Nye Regional Medical Center where they received 3 staples. The employee was released to full duty.

As an employee was installing a wheel group on a haul truck, the pry bar they were using slipped and contacted the employee in the face, causing a laceration. The employee was transported to Nye Regional Medical Center where they received stitches.

An ore process employee was cleaning a discharge chute with a spader. The spader caught his hand in a pinch point.

DURING THE ASSEMBLY OF A BEARING INTO A CYLINDER BORE, THE BEARING BECAME LODGED. A 2X4 & HAMMER WERE BEING USED TO DISLODGE IT. THE 2X4 SPLIT CATCHING THE MECHANICS HAND IN A PINCH POINT.

Attempting to replace by idler wheel on a D-10 dozer, the mechanic got his finger in a pinch point between the idler wheel and a hydrautic jack.

2 EE'S WERE KNOCKING A BEARING RACE OUT WITH A 4X4 BOARD & A HAMMER. THE BOARD SLIPPED & PINCHED THE EE'S THUMB BETWEEN THE BOARD & A METAL BRACE.

A PIT BLAST HOLE DRILLER WAS ATTEMPTING TO DESTROY A DAMAAGED NYLON CHOKER BY HITTING IT TO CUT IT IN 1/2 WITH A DRILL STEEL. THE CHOKER SLIPPED AND THE DRILLER HIT HIS HAND.

WHILE REMOVING A BEARING RACE, THE EMPLOYEE HIT A CHISEL AND A PIECE OF THE RACE CHIPPED OFF AND WENT INTO HIS HAND. SURGERY WAS DONE ON 7/25. THIS WAS TREATED AS A FIRST AID UNTIL JULY WHEN THE PIECE OF METAL WAS LOCATED IN HIS HAND BY AN X-RAY.

THE MECHANIC WAS REPLACING SKIRTING ON THE TAIL ON A CONVEYER BELT. HE WAS USING A RAZOR KNIFE TO CUT THE BELTING AND THE KNIFE SLIPPED AND LACERATED HIS KNEE.

TRUCK DRIVER WAS TRYING TO COUPLE TWO HOSES TOGETHER. HE WAS HITTING THE FITTING WITH A HAMMER WHEN HE MISSED AND HIT HIS LEFT HAND. THE BLOW FROM THE HAMMER CAUSED A CONTUSION AND BROKE A BON E IN HIS HAND.

MECAHNIC WAS REMOVING TIE WRAPS WITH A UTILITY KNIFE, THE KNIFE CUT ONE OF THE WRAPS, IT SLIPPEDAND CUT HIS LEFT WRIST.

THE EE RECIEVED A LACERATION TO HIS RT. LEG. THEEE WAS USING A 6 TON COME-ALONG TO REPOSITION A CRUSHER FEEDER WHEN THE 1" EYE BEING USED AS AN ANCHOR POINT BROKE.

EMPLOYEE WAS ATTEMPTING TO REMOVE A LOCKNUT FROM A WIRING HARNESS AND THE WRENCH SLIPPED OFF. HIS HAND TRAVELED FORWARD AND MADE CONTACT WITH AN AIR LINE FITTING AND CUT THE BACK OF HIS HAND. STITCHES WERE USED TO TREAT THE WOUND.

EE WAS ATTEMPTING TO REMOVE A DRAIN PLUG ON A HAUL TRUCK. PLUG WAS TOO TIGHT SO EE USED A HAMMER TO TAP THE WRENCH. HAMMER MISSED WRENCH AND THE FINGER WAS SMASHED BETWEEN THE WRENCH AND HAMME R HANDLE WHICH CAUSED A FRACTURE TO HIS FINGER.

SDX-200 TONGS WERE SET ON A LARGE OPEN END WRENCH. THEY SLID OFF AND PINCHED HIS FOOT AGAINST THE PLATFORM. WHICH RESULTED IN A CLOSED IN PLACE FRACTURE IN HIS LEFT FOOT.

EMPLOYEE WAS INSTALLING PLASTIC WIRE TIES AND THEN USING A UTILITY KNIFE TO CUT THE ENDS OFF. THE UTILITY KNIFE SLIPPED AND CUT THE EMPLOYEE'S FINGER IN THE PROCESS.

WHILE REPLACING THE PRIMARY SCREEN PANELS THE OPERATOR WAS USING A SLEDGE HAMMER TO SEAT THE SCREEN IN PLACE. ON THE DOWN WARD MOTION THE HAMMER STRUCK THE CROSS MEMBER THEN GLANCED BACK. THIS THIS ACTION PINCHED THE OPERATOR'S FINGER BETWEEN THE HAMMER AND THE SCREEN PANEL.

THE MECHANIC WAS REMOVING A TRANSMISION FILTER HOUSING WITH A RATCHET WRENCH. WHEN THE BOLT CAME LOOSE, THE MECHANIC'S ELBOW STRUCK THE TRUCK'S FRAME CAUSING A SMALL FRACTURE TO THE ELBOW.

EE WAS ATTEMPTING TO CUT A CARD BOARD ROLL WITH A UTILITY KNIFE. THE KNIFE BLADE BROKE AND THE EE'S HAND CONTINUED MOTION. THE EE'S FINGER THEN MADE CONTACT WITH THE BROKEN BLADE WHICH WAS STU CK IN THE CARDBOARD ROLL.
